Indications

Irofol is indicated for the prevention and treatment of iron deficiency anemia, especially during pregnancy, and for preventing megaloblastic anemia in pregnancy.

Pharmacology

Iron is essential for forming hemoglobin and supporting body functions. Iron deficiency anemia often results from chronic bleeding, poor diet, pregnancy, or iron absorption issues. Folic acid deficiency during pregnancy causes megaloblastic anemia. Iron and folic acid together treat anemia and help prevent complications. Iron and folic acid are absorbed in the small intestine, and iron helps in forming blood cells in the bone marrow, where Irofol’s used to make hemoglobin.

Dosage & Administration

Adults: Take one tablet daily, or two tablets daily if needed. During pregnancy, start taking the tablet at the first antenatal visit and continue for three months after delivery. Irofol is best to take the tablet before meals, but if there are stomach issues, Irofol can be taken after meals.

Interaction

Iron absorption is reduced when taken with tetracycline. If both are needed, take iron 3 hours before or 2 hours after tetracycline. Antacids and tea can reduce iron absorption. Iron also seems to lower the effectiveness of penicillamine, and co-trimoxazole may inhibit red blood cell formation. Folic acid may lower anticonvulsant levels in the blood.

Contraindications

Irofol is contraindicated in patients with pernicious anemia or anemia not caused by iron deficiency. The cause of anemia should be determined. The absorption of iron and folic acid is reduced by magnesium trisilicate and antacids with carbonate.

Side Effects

Possible side effects include nausea, vomiting, stomach issues, constipation, and diarrhea.

Pregnancy & Lactation

Irofol is not recommended during the first trimester of pregnancy. Most pregnant women do not need extra folic acid, but if anemia develops despite using this supplement, further investigation is needed. Some patients who had stomach surgery may not absorb iron well. Caution is needed for those with peptic ulcers.

Precautions & Warnings

Care is needed when administering to patients with iron overload conditions, such as hemachromatosis or anemia. If treatment is not effective, other causes of anemia should be checked.

Overdose Effects

If an overdose occurs, gastric lavage should be performed if possible within four hours. A chelating agent like desferrioxamine should be administered. In the meantime, milk or a 5% sodium bicarbonate solution can help neutralize the excess iron.
