Indications

Sedil is used for short-term treatment of mild to moderate anxiety, agitation, fear, and aggression. Sedil helps with anxiety caused by stress, physical symptoms of anxiety, alcohol withdrawal, seizures, as a pre-surgery medication, and for sleep problems in hospitalized patients.

Pharmacology

Sedil works by attaching to a specific part of the GABA receptor and enhancing the effects of GABA, which helps open channels that allow chloride ions to enter cells.

Sedil is quickly absorbed when taken orally, and Sedil reaches its highest concentration in the bloodstream within 15-90 minutes. The average time Sedil stays in the body is 30 hours. Sedil binds to plasma proteins 98-99%. Sedil is metabolized in the liver, and only small amounts of the unchanged drug are excreted in urine. After using a suppository, Sedil is absorbed well and reaches its peak in 1.5-2 hours.

Dosage & Administration

Oral:
For anxiety: 2 mg three times a day, which can be increased to 15-30 mg per day as needed. For the elderly or weak patients, use half the usual dose.
For insomnia due to anxiety: 5-15 mg at bedtime.
For night terrors or sleepwalking in children: 1-5 mg at bedtime.

IM/IV Injection (slow infusion, less than 5 mg per minute):
For severe anxiety, panic attacks, or alcohol withdrawal: 10 mg, repeat if necessary after at least 4 hours.
For febrile convulsions in children: 250 mcg/kg as a slow IV injection.

Rectal (children):
A dose of 500 mcg/kg (up to 10 mg), repeat if needed.

Interaction

Sedil should not be taken with alcohol. Sedil may cause more sedation when used with antipsychotics, sleeping pills, sedative antihistamines, or other drugs that slow the central nervous system like general anesthetics, painkillers, or antidepressants. Sedil can be affected by other medications like phenobarbital, cimetidine, omeprazole, and isoniazid.

Contraindications

Sedil should not be used in patients with myasthenia gravis, lung problems, breathing issues, or if allergic to benzodiazepines.

Side Effects

Sedil is usually well tolerated, but higher doses may cause drowsiness, dizziness, confusion, and difficulty with coordination.

Pregnancy & Lactation

Sedil crosses the placenta and can be found in breast milk, so Sedil should be avoided during pregnancy and breastfeeding if possible. Sedil is classified as category D by the FDA for pregnancy.

Precautions & Warnings

Sedil should not be used for long periods, and stopping Sedil suddenly should be avoided. Caution is advised in patients with breathing problems, muscle weakness, a history of drug or alcohol abuse, or liver/kidney disease.

Overdose Effects

Overdose may cause excessive sedation, muscle weakness, deep sleep, or unusual excitement. Severe cases can lead to loss of coordination, low blood pressure, difficulty breathing, and in rare cases, coma or death.

Therapeutic Class

Sedil belongs to the class of benzodiazepines, which are sedative medications, as well as centrally acting muscle relaxants and primary anti-epileptic drugs.

Store in a cool (below 25°C) and dry place, away from light.
