Indications
It is used to treat erectile dysfunction (ED).
Pharmacology
It is a selective inhibitor of an enzyme called PDE5, which helps in the process of achieving an erection. When a man is sexually stimulated, a chemical called nitric oxide is released in the penis. This chemical helps create a substance called cGMP, which relaxes the smooth muscles in the penis and increases blood flow, causing an erection. By inhibiting PDE5, it allows more cGMP to remain in the body, improving erectile function. However, it works only with sexual stimulation and has no effect without it.
Dosage & Administration
Recommended Dose: 10 mg taken orally about 60 minutes before sexual activity. The dose can be adjusted between 5 mg and 20 mg based on how well it works and if there are any side effects. The maximum recommended dose is once a day. It can be taken with or without food.
For Elderly Patients: For people over 65 years old, the starting dose should be 5 mg.
For Patients with Kidney Problems: No dose adjustment is needed for patients with kidney problems.
For Patients with Liver Problems: Patients with moderate liver issues should start with 5 mg and not exceed 10 mg. It should not be used in patients with severe liver problems.
Interaction
It can increase the blood pressure-lowering effect of medications for high blood pressure. It should be used cautiously when taken with other medicines that lower blood pressure, like alpha blockers. Alcohol does not make its effect stronger. Medicines that affect the liver, like ketoconazole, indinavir, ritonavir, and erythromycin, may slow down how quickly it is removed from the body.
Contraindications
It should not be taken with nitrates or nitric oxide donors because it may increase the risk of low blood pressure. It is also not for people who are allergic to it or any of its ingredients.
Side Effects
The most common side effects include headache, flushing, stuffy or runny nose, upset stomach, indigestion, dizziness, and back pain.
Pregnancy & Lactation
It is not meant for women.
Precautions & Warnings
Doctors should consider a patient’s heart condition before prescribing this medicine because there can be some risks associated with sexual activity. It should not be used by men who are advised against sexual activity due to heart conditions.
Overdose Effects
The highest dose studied in humans was 120 mg, and side effects like blurred vision and back pain were common. Doses up to 80 mg and multiple doses of 40 mg daily for 4 weeks did not cause serious effects. If an overdose occurs, contact a doctor immediately.
