Indications
Amisol Gold IV is used as a source of amino acids, glucose, and electrolytes for both adult and pediatric patients who need IV nutrition. Amisol Gold IV is especially helpful for patients who have basic amino acid needs.
Pharmacology
Amisol Gold IV provides a solution of amino acids to help the body build proteins and heal wounds. Amisol Gold IV also helps reduce the breakdown of proteins in the body. When combined with dextrose, electrolytes, vitamins, and other nutrients, Amisol Gold IV is used for total parenteral nutrition (TPN). Amisol Gold IV can also be given through a peripheral vein along with dextrose and maintenance electrolytes.
Dosage & Administration
Adults: The amount of nitrogen required to maintain body protein depends on the patient’s condition. For no or mild metabolic stress and a normal nutritional state, 0.10-0.15g of nitrogen per kg of body weight per day is needed. For moderate metabolic stress, 0.15-0.20g nitrogen per kg/day is needed. For severe catabolism, such as in burns or trauma, up to 0.20-0.25g nitrogen per kg/day is required. For these needs, the dosage is 11-27 ml per kg per day. In obese patients, the dose should be based on their ideal weight. 1000-2000 ml may be given intravenously over 24 hours, at a rate not exceeding 500 ml in 3 hours (about 30-60 drops per minute).
Infants and children: For infants and children, the maximum infusion rate should be 30 ml per kg of body weight per day. The rate can be gradually increased during the first week of treatment.
Interaction
At the recommended dosage, Amisol Gold IV is not expected to have any pharmacological effects or interact with other medications.
Contraindications
Amisol Gold IV is not recommended for patients with certain inherited amino acid metabolism disorders, severe liver damage, or severe uremia without dialysis. Amisol Gold IV should also not be used in patients with hyperosmolar nonketotic diabetic coma due to its glucose content. Additionally, Amisol Gold IV is contraindicated for patients allergic to any of its ingredients.
Side Effects
Amisol Gold IV is generally well tolerated, but side effects can include rare nausea, vomiting, flushing, and sweating when infused too quickly. Some patients may experience mild increases in liver test results. Hypersensitivity reactions and thrombophlebitis (inflammation of the vein) may occur, especially when used in peripheral veins. The incidence of thrombophlebitis may be reduced if fat emulsions are infused at the same time.
Pregnancy & Lactation
Amisol Gold IV has been safely used during pregnancy in humans, but animal studies have not been conducted. If pregnant or breastfeeding, consult with a physician before use.
Precautions & Warnings
Amisol Gold IV should be used cautiously in patients with certain conditions like diabetes, heart failure, or kidney problems. In premature infants, Amisol Gold IV is important to monitor levels of phenylalanine and adjust the infusion rate if needed. If a solution is cloudy or contains particles, do not use Amisol Gold IV and discard any unused portion.
