Indications
Phenoxymethyl is used to treat mild to moderate bacterial infections caused by germs that can be treated with oral penicillin. These include:
- Infections in the ear, nose, and throat, like tonsillitis, pharyngitis, laryngitis, otitis media, and sinusitis.
- Infections in the lungs, like bronchitis and pneumonia.
- Infections caused by group A streptococcus bacteria, like scarlet fever, erysipelas, and rheumatic fever.
- Skin infections like pyodermia, furunculosis, and erythema migrans, if the germs are sensitive to penicillin.
- Infections of the lymph nodes and lymphatic system.
- Infections in the mouth, gums, or jaws, such as delayed teeth growth or secondary bacterial infections following viral gum infections.
Phenoxymethyl is also used to prevent scarlet fever, stop recurring rheumatic fever, and to prevent infections after dental procedures, especially for high-risk patients (e.g., those with heart problems or artificial heart valves).
Pharmacology
Phenoxymethyl is a type of penicillin that works well in treating infections caused by Gram-positive bacteria. Phenoxymethyl is absorbed well from the small intestine, and Phenoxymethyl works best when taken 1 hour before or 2 hours after meals. Phenoxymethyl is more stable in the stomach compared to other penicillins.
Dosage & Administration
Adults: 250-500 mg every 6 hours.
Children (above 1 year): 125-250 mg every 6 hours.
125 mg/5 ml suspension: 1-2 teaspoonfuls (5-10 ml) every 6 hours.
250 mg/5 ml suspension: ½-1 teaspoonful (2.5-5 ml) every 6 hours.
Infants (below 1 year): 62.5-125 mg every 6 hours.
125 mg/5 ml suspension: ½-1 teaspoonful (2.5-5 ml) every 6 hours, or as directed by the doctor.
Phenoxymethyl is best taken on an empty stomach, at least 1 hour before or 2 hours after meals.
Interaction
Taking Phenoxymethyl with food may lower how well the body absorbs Phenoxymethyl. Phenoxymethyl should be taken on an empty stomach for best results.
Drug Interactions: When taken with methotrexate, Phenoxymethyl may increase the levels of methotrexate in the blood and cause side effects. Phenoxymethyl should not be used with tetracyclines, chloramphenicol, or other bacteriostatic antibiotics because Phenoxymethyl may not work as well.
Phenoxymethyl can affect the absorption of other medications, and Phenoxymethyl may interact with drugs like probenecid, indomethacin, and warfarin, causing increased or decreased levels of the drug in the blood.
Contraindications
Phenoxymethyl is not for people who are allergic to penicillin or any of its ingredients. Phenoxymethyl should not be used in patients with serious stomach issues like vomiting or diarrhea.
Side Effects
Some side effects include skin rashes, itching, fever, joint pain, stomach issues like nausea, vomiting, and diarrhea, and in rare cases, severe reactions like anaphylaxis or skin conditions like Stevens-Johnson syndrome. If any of these happen, stop taking the medication and seek medical help.
Phenoxymethyl can also cause changes in blood cell counts, which may require medical attention.
Pregnancy & Lactation
Phenoxymethyl can be used during pregnancy if needed. Phenoxymethyl also passes into breast milk, but Phenoxymethyl is safe to use while breastfeeding.
Precautions & Warnings
If you are allergic to cephalosporins, you may also be allergic to penicillin. Be careful if you have heart disease or electrolyte problems because the potassium content may need to be considered. If you have diabetes, the syrup may contain sugar.
Using this medication for a long time may cause infections that are resistant to treatment, so your doctor may monitor your condition regularly.
Overdose Effects
Overdose is rare and usually does not cause serious harm. However, Phenoxymethyl can cause confusion or seizures in cases of overdose, especially if the kidneys are not working well. If overdosed, contact your doctor immediately.
