Indications
It is used to relieve symptoms of osteoarthritis and rheumatoid arthritis. Before using it, consider the potential benefits and risks compared to other treatment options. Always use the lowest effective dose for the shortest duration as per individual treatment goals.
Pharmacology
It is a non-steroidal anti-inflammatory drug (NSAID) with anti-inflammatory, pain-relieving, and fever-reducing properties. Although its exact mechanism of action is not fully understood, it may work by inhibiting the production of prostaglandins. It is a prodrug that gets converted in the liver into an active component, which strongly inhibits prostaglandin synthesis.
Dosage & Administration
Adult & Elderly: Start with 1000 mg once a day, with or without food. Some patients may need 1500 mg to 2000 mg per day for better relief. It can be taken once or twice daily. Do not exceed 2000 mg per day. Use the lowest effective dose for long-term treatment.
Moderate or severe kidney problems: In patients with moderate or severe renal insufficiency, the starting dose should not exceed 750 mg or 500 mg once a day, respectively. The dose may be increased to a maximum of 1500 mg and 1000 mg daily, with careful monitoring of kidney function.
Children: It is not recommended for children as its safety and effectiveness in pediatric patients have not been established.
Interaction
It may reduce the blood pressure-lowering effect of ACE inhibitors. Patients using both medicines should discuss this with their healthcare provider.
Contraindications
It should not be used by people who are allergic to it or its ingredients. Avoid using it if you have experienced allergic reactions such as asthma or hives after taking aspirin or other NSAIDs. It is also not recommended for use in patients undergoing coronary artery bypass surgery (CABG).
Side Effects
Gastrointestinal: Diarrhea, upset stomach, abdominal pain, constipation, bloating, nausea, dry mouth, stomach inflammation, mouth sores, vomiting.
Central Nervous System: Dizziness, headache, fatigue, sweating, insomnia, nervousness, drowsiness.
Skin: Itching, rash.
Special Senses: Ringing in the ears.
Miscellaneous: Swelling.
Pregnancy & Lactation
It is classified as Pregnancy Category C. There are no well-controlled studies in pregnant women. It should only be used during pregnancy if absolutely necessary, especially in the third trimester due to its effect on fetal cardiovascular development. It is not recommended for nursing mothers because it may affect the baby.
Precautions & Warnings
NSAIDs can cause kidney problems, especially in people with conditions like kidney disease, heart failure, liver problems, or the elderly. Careful monitoring is required when using this drug long-term. Stopping the medication usually allows kidneys to recover.
Overdose Effects
Symptoms of overdose can include drowsiness, nausea, vomiting, stomach pain, and lethargy, but these are usually temporary and can be treated with supportive care. Severe overdose may cause high blood pressure, kidney failure, difficulty breathing, and even coma, although this is rare. Treatment includes supportive care and may include activated charcoal if the overdose occurred within 4 hours of ingestion.
