Indications
Febus 40 is used for managing high uric acid levels in people with gout. Febus 40 is not recommended for treating high uric acid levels in people without symptoms.
Pharmacology
Febus 40 is a medicine that works by blocking an enzyme called xanthine oxidase. This enzyme breaks down substances in the body that produce uric acid. By blocking this enzyme, Febus 40 lowers the level of uric acid in the blood. Febus 40 does not affect other processes in the body related to purine or pyrimidine, which are important for cell function.
Dosage & Administration
Starting Dose: One 40 mg tablet once a day. If the uric acid level is still high after two weeks, increase to 80 mg once a day. If needed, increase to 120 mg after another 2-4 weeks.
Tumor Lysis Syndrome: Take 120 mg once daily, starting two days before chemotherapy and continue for at least 7 days. The duration may be extended up to 9 days depending on the chemotherapy.
Gout Flares: Gout flares may happen when starting this medicine. To prevent them, take a non-steroidal anti-inflammatory drug (NSAID) or colchicine. If a flare occurs, continue the treatment and manage the flare separately.
Renal Impairment: No change in dose is needed for mild to moderate kidney problems.
Hepatic Impairment: No change in dose is needed for mild to moderate liver problems.
Pediatric Use: This medicine is not recommended for children under 18, as safety and effectiveness have not been established.
Interaction
Mercaptopurine/Azathioprine: These drugs should not be used together with Febus 40, as Febus 40 may increase their levels in the blood and cause harm.
Rosiglitazone/CYP2C8 Substrates: No dose change is needed when using Febus 40 with these drugs.
Naproxen & Other Glucuronidation Inhibitors: Febus 40 can be taken with naproxen without changing the dose of either drug.
Inducers of Glucuronidation: Drugs that speed up its metabolism might reduce its effectiveness. Febus 40 is recommended to monitor uric acid levels after starting these drugs.
Colchicine/Indomethacin/Other Medications: Febus 40 can be used together with colchicine, indomethacin, hydrochlorothiazide, and warfarin without needing a dose change.
Antacids: Taking Febus 40 with magnesium hydroxide and aluminum hydroxide (found in some antacids) may delay its absorption by about 1 hour but does not affect its effectiveness.
Contraindications
Febus 40 should not be used by patients who are taking azathioprine, mercaptopurine, or theophylline.
Side Effects
The most common side effects include gout flares, liver problems, diarrhea, nausea, headaches, skin rash, and swelling. These effects are usually mild. Serious allergic reactions are rare.
Pregnancy & Lactation
Febus 40 is a Category C drug during pregnancy, which means Febus 40 should only be used if the benefits outweigh the risks. Febus 40 is unknown if Febus 40 passes into breast milk, so Febus 40 should be used cautiously by nursing mothers.
Precautions & Warnings
Gout Flare: Gout flares are common when starting this medicine. They can be treated without stopping the medicine.
Cardiovascular Events: There is a higher risk of heart problems in some patients taking Febus 40. Monitor for signs of heart attack or stroke.
Liver Enzyme Elevation: Febus 40 can cause liver enzyme levels to rise. Regular liver function tests are recommended.
Overdose Effects
In studies, Febus 40 has been given in doses up to 300 mg per day for 7 days without causing significant harmful effects.
