Indications
Macrobid is used to treat the following conditions:
– Acute worsening of chronic bronchitis in adults
– Acute maxillary sinus infection
– Community-acquired pneumonia
– Throat or tonsil infection
– Uncomplicated skin and soft tissue infections
– Acute ear infections in children
– Treatment and prevention of certain bacterial infections
– Helicobacter pylori infection and stomach ulcers in adults
Pharmacology
Macrobid works by stopping the production of proteins in bacteria. Macrobid mainly binds to the bacterial ribosome, stopping them from multiplying. Macrobid is effective against many Gram-positive bacteria, Chlamydia, some Gram-negative bacteria, and Mycoplasmas. Macrobid works better than erythromycin against most Gram-positive bacteria. Macrobid is more stable in the stomach than erythromycin, which makes Macrobid easier to tolerate. Macrobid also has more power than erythromycin against H. influenzae, though most Gram-negative bacteria are resistant to Macrobid.
Dosage & Administration
Adults:
– For acute bacterial bronchitis: 250 mg to 500 mg every 12 hours for 7 to 14 days.
– For acute maxillary sinus infection: 500 mg every 12 hours for 14 days.
– For pneumonia: 250 mg every 12 hours for 7 to 14 days.
– For throat infection: 250 mg every 12 hours for 10 days.
– For uncomplicated skin infections: 250 mg every 12 hours for 7 to 14 days.
– For Mycobacterium avium infections: 500 mg every 12 hours until the patient is at low risk.
– For Helicobacter pylori infection:
– Triple therapy: 500 mg clarithromycin, 30 mg lansoprazole, 1 g amoxicillin, every 12 hours for 10 or 14 days.
– Dual therapy: 500 mg clarithromycin every 8 hours and 40 mg omeprazole once every morning for 14 days.
Children:
– 15 mg/kg per day, divided every 12 hours for 10 days.
– For bacterial infections: 7.5 mg/kg every 12 hours up to 500 mg.
Renal and liver impairment: Adjust dosage if severe renal problems are present.
Interaction
Macrobid should not be used with drugs like statins (e.g., lovastatin), certain antiarrhythmics, or anti-gout medications (e.g., colchicine). Macrobid can cause dangerous interactions with some other medications.
Contraindications
Macrobid should not be used by people who are allergic to clarithromycin, erythromycin, other similar drugs, or any of the ingredients in the product. Macrobid should also not be used by those who have had liver problems due to clarithromycin in the past.
Side Effects
Side effects are usually mild and may include nausea, vomiting, diarrhea, and stomach pain. Other effects like mouth inflammation, headaches, allergic reactions, and taste changes may also occur. Some people may feel anxious, dizzy, or have trouble sleeping.
Pregnancy & Lactation
Macrobid may be used in babies, but mothers taking clarithromycin should not breastfeed until treatment is finished. Macrobid is not recommended for pregnant women due to limited experience.
Precautions & Warnings
If severe allergic reactions happen, Macrobid should be stopped. Macrobid should be avoided in people with heart rhythm problems or low potassium levels. Macrobid should also be discontinued if symptoms of liver problems appear. Patients with heart disease should be careful, as Macrobid may increase the risk of death after treatment ends.
Overdose Effects
If too much is taken, Macrobid can cause stomach issues like nausea and vomiting. Treatment should involve removing the drug from the stomach and supportive care. Dialysis may not significantly reduce the drug levels.

Storage Conditions
