Indications

Abecab 10/40 is used to treat high blood pressure either on its own or with other blood pressure-lowering medicines. Abecab 10/40 can be used as the first treatment for patients who may need multiple medicines to reach their blood pressure target. The decision to use a combination of medicines should be based on individual factors like the patient’s current blood pressure, target goals, and the chance of reaching the goal with a combination compared to a single medicine.

Pharmacology

Amlodipine is a calcium channel blocker that works by relaxing the blood vessels and reducing the pressure. Abecab 10/40 mainly affects the smooth muscles of the blood vessels rather than the heart muscles. Abecab 10/40 helps reduce the resistance in blood vessels, lowering blood pressure.

Olmesartan Medoxomil works by blocking the effects of angiotensin II, a hormone that narrows the blood vessels and increases blood pressure. Abecab 10/40 stops this hormone from binding to its receptors in the blood vessels and adrenal glands. This helps to lower blood pressure. Abecab 10/40 does not affect the enzyme that produces angiotensin II, called ACE.

Dosage & Administration

Starting Dose: Begin with 5/20 mg once a day for 1 to 2 weeks, and adjust if necessary up to a maximum of 10/40 mg daily. For elderly patients, the starting dose of Amlodipine should be 2.5 mg due to slower clearance. This combination drug is not recommended for patients over 75 years old as the lowest dose is 5/20 mg.

Interaction

The effect of this medicine may be reduced if taken with NSAIDs, including selective COX-2 inhibitors. Blood pressure, kidney function, and electrolytes should be carefully monitored for patients taking this combination with other medicines that affect the Renin-angiotensin system.

Contraindications

Abecab 10/40 cannot be used with Aliskiren in patients with diabetes.

Side Effects

The most common side effects are swelling of the legs, headaches, flushing, and dizziness. Abecab 10/40 may also cause a digestive issue known as sprue-like enteropathy, which can lead to problems with the intestines.

Pregnancy & Lactation

This combination should not be used during the second and third trimesters of pregnancy as Abecab 10/40 can harm the fetus. If pregnancy is confirmed, the medication should be stopped immediately. Abecab 10/40 is unclear if the medicine passes into breast milk, so the mother should decide whether to stop breastfeeding or stop the medication, depending on what’s best for her health.

Precautions & Warnings

This combination should be used carefully in patients who may experience low blood pressure, those with severe narrowing of the heart valve, or those with serious heart problems. Abecab 10/40 should also be used with caution in patients with conditions that can lead to higher blood pressure, like salt or fluid loss.
