Indications
Acmecilin is used to treat infections caused by certain bacteria, including those in the genitourinary tract (like gonorrhea), respiratory tract, gastrointestinal tract, and meningitis. Acmecilin can also be used for infections like shigella, salmonella, and others, and should be used based on bacterial testing results.
Pharmacology
Acmecilin works by blocking the bacteria’s ability to build its cell wall, which leads to the bacteria breaking apart. This happens because the cell wall is no longer able to form properly, causing the bacteria to be destroyed.
Dosage & Administration
For intra-articular use: For adults, 500 mg daily; for children under 10 years old, half the adult dose.
For intraperitoneal use: For adults, 500 mg daily; for children under 10 years old, half the adult dose.
For intrapleural use: For adults, 500 mg daily; for children under 10 years old, half the adult dose.
For intravenous use in meningitis: For adults, 2 gm every 6 hours; for children, 150 mg/kg daily in divided doses.
For oral use in infections like bronchitis, otitis media, and urinary tract infections: For adults, 0.25-1 gm every 6 hours; for children under 10 years old, half the adult dose.
For typhoid fever: For adults, 1-2 gm every 6 hours for 2 weeks; for carriers, 4-12 weeks.
For uncomplicated gonorrhea: For adults, 2 gm with 1 gm of probenecid as a single dose.
For septicaemia: For adults, 150-200 mg/kg daily, starting with IV for at least 3 days and then continuing with IM injections. Treatment should last 48-72 hours after symptoms go away.
Interaction
Acmecilin may make oral contraceptives less effective, affect blood clotting when taken with warfarin, and reduce the effectiveness of oral typhoid vaccines. Acmecilin can also increase the risk of side effects when taken with certain medications like methotrexate, probenecid, and sulfinpyrazone. Some antibiotics may reduce its effectiveness.
Contraindications
Acmecilin should not be used by people who are allergic to ampicillin or other penicillin antibiotics.
Side Effects
Side effects can include nausea, vomiting, diarrhea, rashes, sore mouth, fever, joint pain, anemia, low blood platelets, and liver or kidney issues. Rarely, Acmecilin can cause serious allergic reactions and infections like Clostridium difficile-associated diarrhea (CDAD).
Pregnancy & Lactation
Acmecilin is generally considered safe during pregnancy, but you should consult a doctor before use, especially in the first trimester.
Precautions & Warnings
People with a history of allergic reactions to β-lactam antibiotics should be cautious. Those with kidney problems, or who are pregnant or breastfeeding, should also take care.
Overdose Effects
Symptoms of overdose may include nausea, vomiting, and diarrhea. Treatment involves supportive care, and the drug can be removed from the body through hemodialysis.
