Indications
Acora is used with acetylsalicylic acid (ASA) to prevent blood clots in adults who have had a heart attack or have a high risk of having another heart attack or stroke.
Pharmacology
Acora works by blocking a receptor on platelets in the blood, which helps prevent them from clumping together and forming harmful clots. This helps reduce the risk of heart attacks and strokes.
Dosage & Administration
Adults: Start with a 180 mg dose (2 tablets of 90 mg), then continue with 90 mg twice a day. For 12 months, continue with twice-daily doses unless a doctor advises otherwise.
Heart Attack History: For patients with a history of heart attack and a high risk of another one, the dose is 60 mg twice daily after one year. Treatment can start even up to 2 years after a heart attack or after stopping another treatment.
Missed Dose: If a dose is missed, take the next scheduled dose as usual.
Older Adults: No dose adjustment needed.
Kidney or Liver Issues: No dose adjustment is needed for kidney problems, but Acora is not recommended for patients on dialysis. Acora is not recommended for patients with severe liver problems and should be used with caution for mild to moderate liver issues.
Interaction
Avoid using Acora with strong CYP3A4 inhibitors (e.g., clarithromycin) or CYP3A4 inducers (e.g., rifampicin). Acora may affect how other medications like statins and digoxin work. Use caution when combining with medications that affect blood clotting, such as heparin.
Contraindications
Acora should not be used by people who are allergic to Acora or any of its ingredients, have active bleeding, a history of brain hemorrhage, or severe liver problems.
Side Effects
Common side effects include bleeding, shortness of breath, dizziness, headache, low blood pressure, and stomach problems like nausea and diarrhea. Serious side effects like brain or eye bleeding are rare but can happen.
Pregnancy & Lactation
Acora is not recommended during pregnancy. If breastfeeding, consult a doctor because Acora can pass into breast milk, and the risk to the baby is not clear.
Precautions & Warnings
Acora should be used with caution in people who have a higher risk of bleeding, such as those with recent surgery or gastrointestinal bleeding. Patients should stop Acora 5 days before surgery if needed. Acora should be used carefully in people with asthma or lung disease, as Acora can cause shortness of breath. Regular checks for kidney function and uric acid levels are also advised.
Overdose Effects
Overdose can cause stomach upset, shortness of breath, and heart issues like slowed heart rate. Seek immediate medical attention if overdose is suspected.
