Indications

Actical is used to prevent or treat low calcium levels in the blood for people who do not get enough calcium from their diet. Actical may be used to treat conditions caused by low calcium, like bone loss (osteoporosis), weak bones (osteomalacia, rickets), decreased activity of the parathyroid gland (hypoparathyroidism), and some muscle diseases (latent tetany). Actical may also be used to ensure people are getting enough calcium, especially after menopause.

Pharmacology

Calcium is essential for normal functioning of nerves, cells, muscles, and bones. When the blood has low calcium, the body takes calcium from the bones, which weakens them. Calcium citrate is absorbed well even on an empty stomach and does not cause constipation. Actical is less affected by stomach acid than other types of calcium.

Calcitriol is the active form of vitamin D3. Actical helps the body absorb calcium and phosphate from the intestines and is important for bone mineralization. In chronic kidney disease, the body does not produce enough calcitriol, which causes problems with calcium and phosphate levels in the blood. Calcitriol helps to correct this by improving calcium absorption and reducing bone disease symptoms.

In patients with post-menopausal osteoporosis, Calcitriol improves calcium absorption and reduces the chance of spine fractures.

Dosage & Administration

1-4 tablets per day with or without food, depending on the doctor’s instructions.

Interaction

Calcium may reduce the absorption of tetracyclines, quinolones, and oral bisphosphonates. Corticosteroids may reduce calcium absorption. Taking thiazide diuretics with calcium may increase the risk of high calcium levels and metabolic alkalosis. High calcium levels may increase the effect of heart medications like digoxin. Calcium can reduce the effectiveness of erlotinib. Using calcium with paricalcitol increases the risk of high calcium levels. Certain anti-seizure medications can speed up vitamin D breakdown. If calcium citrate is taken with aluminum-based products in people with kidney disease, Actical can cause dangerous increases in aluminum levels.

Contraindications

Actical should not be used by people with high calcium levels in the blood (hypercalcaemia), high calcium in urine (hypercalciuria), or low phosphate levels (hypophosphatemia).

Side Effects

Side effects may include constipation, nausea, stomach pain, high calcium levels (hypercalcaemia), high calcium in urine (hypercalciuria), headache, and muscle weakness.

Pregnancy & Lactation

Actical is classified as pregnancy category C. Actical should be used during pregnancy and breastfeeding only as directed by a doctor.

Precautions & Warnings

Use with caution in people with high calcium levels, malabsorption syndrome, heart disease, kidney disease, kidney stones, certain immune system disorders (such as sarcoidosis), liver disease, and certain bowel diseases (like Crohn’s disease).
