Indications
Actilyse is used to treat sudden strokes caused by blocked blood flow, heart attacks to reduce the risk of death and heart failure, and severe lung blood clots that affect breathing and blood circulation.
Pharmacology
Actilyse helps break down blood clots by converting an inactive protein into an active one, which dissolves the clot. Actilyse mainly works at the site of the clot, limiting effects on the rest of the body.
Dosage & Administration
Stroke: Give within 3 hours of symptoms. Dose: 0.9 mg/kg (max 90 mg), with 10% given as a quick injection and the rest slowly over an hour.
Heart Attack: Give as soon as possible. Dose depends on body weight but does not exceed 100 mg.
Severe Lung Clots: Dose: 100 mg given over 2 hours with blood thinners started near the end.
Interaction
Actilyse may interact with blood thinners and medicines affecting clotting, increasing the risk of bleeding. Close monitoring is needed when used with other anticoagulants.
Contraindications
Actilyse should not be used in people with active internal bleeding, recent major surgery, brain bleeding, or those with a high risk of severe bleeding disorders.
Side Effects
Common side effects include bleeding, low blood pressure, fever, nausea, and headache. Severe bleeding in the brain is a rare but serious risk.
Pregnancy & Lactation
Use during pregnancy only if the benefits outweigh the risks. If breastfeeding, consult a doctor before use as Actilyse is not clear if the medicine passes into breast milk.
Precautions & Warnings
Care is needed in patients with recent surgery, uncontrolled high blood pressure, or those prone to bleeding. Regular monitoring is required.
Overdose Effects
Overdose may lead to excessive bleeding, low blood pressure, and shock. Immediate medical attention is required in case of overdose.
