Indications
Acumet 50 is used to manage high blood pressure and chest pain (angina). Acumet 50 helps control heart rhythm problems, especially those caused by fast heartbeats. Acumet 50 is also used as an additional treatment for hyperthyroidism. Early use of Acumet 50 in heart attacks can reduce heart damage and prevent dangerous heart rhythms. Acumet 50 may also reduce the need for strong pain medications.
Pharmacology
Acumet 50 is a selective beta1-blocker that helps to reduce the effects of stress on the heart. Acumet 50 slows down the heart rate and lowers blood pressure by blocking certain chemicals in the body. Acumet 50 affects insulin release and carbohydrate metabolism less than other beta-blockers. Acumet 50 also has a smaller impact on blood sugar and heart response to low blood sugar compared to other beta-blockers.
Dosage & Administration
Film-coated tablet:
For high blood pressure: The usual dose is 100-400 mg per day, either in one or two doses. The starting dose is 100 mg per day, which may be increased weekly. For chest pain (angina): Usually, 50 mg to 100 mg is taken two or three times a day.
Cardiac arrhythmias: The usual dose is 50 mg twice or three times daily, and may be increased up to 300 mg per day.
For hyperthyroidism: 50 mg four times a day, with dose adjustments as needed.
After a heart attack: Therapy starts with 50 mg every 6 hours for 48 hours, followed by a maintenance dose of 200 mg daily in divided doses.
Extended-release tablet:
For high blood pressure and angina: The usual starting dose is 25-100 mg daily.
For heart failure: The recommended starting dose is 25 mg daily for two weeks, which can be increased gradually.
Interaction
Acumet 50 may have stronger effects when taken with certain drugs like Reserpine or MAO inhibitors. Drugs that affect the liver enzyme CYP2D6, such as quinidine, fluoxetine, and propafenone, can increase the levels of Acumet 50 in the body. Using Acumet 50 with digitalis glycosides can slow the heart rate, and stopping clonidine suddenly can cause high blood pressure.
Contraindications
Acumet 50 should not be used in patients with heart block, severe heart failure, very slow heart rate, or severe peripheral artery disease. Acumet 50 should also be avoided if there is an allergy to Acumet 50 or other beta-blockers, or if a heart attack is complicated by severe slow heart rate or very low blood pressure.
Side Effects
Possible side effects include tiredness, dizziness, depression, nausea, slow heart rate, shortness of breath, and digestive issues. Acumet 50 can also cause confusion, headaches, sleep problems, muscle pain, or dry mouth.
Pregnancy & Lactation
Acumet 50 is classified as pregnancy category C, meaning Acumet 50 should be used only if necessary during pregnancy. Acumet 50 is found in small amounts in breast milk, so caution is needed when using Acumet 50 while nursing.
Precautions & Warnings
Acumet 50 should be used with caution in patients with breathing problems. Acumet 50 may also affect how the heart responds to stress during surgery. Acumet 50 can mask signs of low blood sugar in people with diabetes and may worsen symptoms in patients with poor circulation.
Overdose Effects
Overdose symptoms may include very low blood pressure, slow heart rate, heart failure, or even coma. Treatment may involve monitoring heart and lung function, giving medications to increase heart rate, and using devices like pacemakers. If too much is taken, Acumet 50 should be removed from the stomach if recent, and other treatments like glucagon or specific inhalers may be needed.
