Indications

Adair is used for the long-term treatment of severe chronic obstructive pulmonary disease (COPD) with chronic bronchitis in adults who have frequent flare-ups. Adair is added to bronchodilator therapy.

Pharmacology

Adair is a PDE-4 inhibitor that works by blocking the PDE4 enzyme. This increases the levels of cyclic AMP in the cells, which helps reduce inflammation and improves lung function.

Dosage & Administration

Adult dose: Take one 500 microgram tablet once daily. Adair may take several weeks for the medicine to show its effect. Adair has been tested for up to one year in clinical studies.

Elderly (65 years and older): No change in dose is needed.

Renal impairment: No dose adjustment is necessary.

Hepatic impairment: For mild liver problems (Child-Pugh A), use with caution. Adair should not be used in patients with moderate or severe liver problems (Child-Pugh B or C).

Pediatric population: Adair is not recommended for use in children under 18 years old.

The tablet should be taken with water at the same time every day, with or without food.

Interaction

There are no major dangerous interactions. However, there are reports of interactions with alcohol and other drugs like pyrimethamine with dapsone, and with betahistine and salbutamol. Taking these together might reduce their effects.

Contraindications

Adair should not be used by people who are allergic to Adair or any of its ingredients, or those with moderate or severe liver problems.

Side Effects

Common side effects in COPD studies include diarrhea (5.9%), weight loss (3.4%), nausea (2.9%), abdominal pain (1.9%), and headache (1.7%). Most side effects are mild to moderate and occur in the first few weeks of treatment, typically going away as treatment continues.

Other possible side effects:

  • Immune system: Uncommon hypersensitivity reactions.
  • Endocrine: Rare gynaecomastia.
  • Metabolism: Common weight loss, decreased appetite.
  • Psychiatric: Common insomnia, rare depression, anxiety, and suicidal thoughts.
  • Nervous system: Common headache, rare tremor, dizziness, or loss of taste.
  • Cardiac: Rare palpitations.
  • Respiratory: Rare respiratory infections.
  • Gastrointestinal: Common diarrhea, nausea, abdominal pain, and rare constipation.
  • Liver: Rare increased liver enzymes.
  • Skin: Uncommon rash, rare hives.
  • Musculoskeletal: Uncommon muscle weakness and pain.
  • General: Uncommon fatigue and malaise.

Pregnancy & Lactation

Pregnancy: There is limited information about its use during pregnancy. Animal studies show Adair can harm the fetus. Adair is not recommended for pregnant women or those planning to get pregnant.

Lactation: Roflumilast and its metabolites can pass into breast milk, which might harm a baby. Adair should not be used while breastfeeding.

Precautions & Warnings

Patients should be aware of the risks of using Roflumilast and follow safety guidelines.

Rescue medications: Roflumilast is not for treating sudden breathing problems.

Weight loss: Some patients may experience weight loss. Regular weight checks are recommended. If weight loss is significant, treatment should be stopped.

Special conditions: Adair should not be used in patients with severe immunological diseases, serious infections, or certain cancers. Adair is also not suitable for those with severe heart failure or active infections like tuberculosis or viral hepatitis.

Psychiatric disorders: Roflumilast may increase the risk of mental health issues like insomnia, anxiety, or depression. Rare cases of suicidal thoughts or behavior have been reported.

Overdose Effects

Symptoms of an overdose may include stomach pain, nausea, vomiting, diarrhea, and in severe cases, dizziness, low blood pressure, or even coma.

Therapeutic Class

Adair belongs to the class of medications known as anti-inflammatory drugs.
