Indications

Adefovir Dipivoxil is used for treating chronic hepatitis B in adults who show active viral replication, along with persistent elevations in serum aminotransferases (ALT or AST) or histologically active disease.

Pharmacology

Adefovir dipivoxil is a prodrug of adefovir, an acyclic nucleotide analog effective against human hepatitis B virus (HBV). Adfovir is phosphorylated to the active metabolite, adefovir diphosphate, by cellular kinases. This metabolite inhibits HBV DNA polymerase (reverse transcriptase) by competing with deoxyadenosine triphosphate and causing DNA chain termination after incorporation into viral DNA. The oral bioavailability of adefovir is approximately 59% from a 10 mg dose. Adfovir binds minimally to plasma proteins (4%) and is excreted primarily through the kidneys.

Dosage & Administration

The recommended dose for chronic hepatitis B patients with adequate renal function is 10 mg once daily, taken orally, regardless of food.

Interaction

The pharmacokinetics of adefovir remain unchanged when coadministered with lamivudine, trimethoprim/sulfamethoxazole, and acetaminophen. However, coadministration with ibuprofen (800 mg three times daily) can lead to increases in adefovir’s Cmax (33%), AUC (23%), and urinary recovery due to higher oral bioavailability.

Contraindications

Adefovir dipivoxil is contraindicated in patients who are hypersensitive to any of its components.

Side Effects

Common side effects include weakness, headache, stomach pain, and nausea. Discontinuation of therapy may result in severe acute exacerbations of hepatitis. In patients with underlying renal dysfunction, prolonged use can lead to nephrotoxicity. Lactic acidosis and severe hepatomegaly with steatosis, including fatal cases, have been reported with nucleoside analogs, alone or in combination with other antiretrovirals.

Pregnancy & Lactation

Pregnancy Category C. Adequate and well-controlled studies in pregnant women are lacking. Adefovir dipivoxil should be used during pregnancy only if clearly needed and after carefully considering the risks and benefits. Adfovir is not known if adefovir is excreted in human milk, so breastfeeding should be avoided while using this medication.

Precautions & Warnings

Patients who stop taking adefovir dipivoxil should be monitored regularly for hepatic function. Close monitoring is also necessary for patients with or at risk of renal dysfunction, and they may need dose adjustments.

Overdose Effects

Overdoses of adefovir dipivoxil (500 mg daily for 2 weeks or 250 mg daily for 12 weeks) have been linked to gastrointestinal side effects. If an overdose occurs, the patient must be monitored for toxicity, and standard supportive treatments should be applied as needed.
