Indications
Adrebloc is used for short-term treatment to control the heart rate in certain heart conditions, such as irregular heartbeats (supraventricular tachycardia) like atrial fibrillation and atrial flutter, and also for non-compensatory sinus tachycardia (fast heart rate). Adrebloc is also used to manage heart rate and blood pressure during surgeries.
Pharmacology
Adrebloc is a cardioselective (beta-1 specific) beta blocker that works quickly and lasts for a short time. Adrebloc has no significant stimulating or stabilizing effects on the heart at normal doses. Adrebloc helps block beta-1 receptors found mainly in the heart. However, at higher doses, Adrebloc may also affect beta-2 receptors in the lungs and blood vessels. The drug’s effects typically wear off in about 9 minutes after an IV dose.
Dosage & Administration
Administer intravenously. The dose can be adjusted based on heart rate or blood pressure, with adjustments made every 4 minutes.
For supraventricular tachycardia (SVT) or non-compensatory sinus tachycardia: Start with an optional loading dose of 500 mcg/kg over 1 minute. Then, give 50 mcg/kg per minute for the next 4 minutes. The dose can be adjusted as needed, but the maximum is 200 mcg/kg per minute.
For perioperative tachycardia and hypertension: Start with a loading dose of 500 mcg/kg over 1 minute for gradual control (or 1 mg/kg over 30 seconds for immediate control). Then, give 50 mcg/kg per minute for gradual control (or 150 mcg/kg per minute for immediate control), adjusting to a maximum of 200 mcg/kg per minute for tachycardia or 300 mcg/kg per minute for hypertension.
Interaction
Adrebloc may interact with:
- Digitalis glycosides: May cause slow heart rate (bradycardia).
- Anticholinesterases: Can prolong neuromuscular blockage.
- Antihypertensive agents: May cause a sudden increase in blood pressure after stopping other blood pressure medications.
- Sympathomimetic drugs: May require a dose adjustment.
- Vasoconstrictors and positive inotropic agents: Avoid using together.
Contraindications
Adrebloc should not be used in people with:
- Severe slow heart rate (sinus bradycardia)
- Heart block greater than first degree
- Sick sinus syndrome
- Severe heart failure
- Cardiogenic shock
- Taking calcium-channel blockers (e.g. verapamil) in close proximity
- Pulmonary hypertension
- Known allergic reactions to this drug
Side Effects
The most common side effects (occurring in more than 10% of people) are low blood pressure (hypotension), sweating, and dizziness. Some people may also experience asymptomatic hypotension (low blood pressure without symptoms).
Pregnancy & Lactation
Adrebloc is classified as Pregnancy Category C. Studies in animals have shown Adrebloc can cause an increase in fetal loss, though the mother’s health was not significantly affected. There are no well-controlled studies in pregnant women. Adrebloc should only be used during pregnancy if the potential benefits outweigh the risks to the baby. Adrebloc is not known whether this drug passes into breast milk, so the decision to discontinue breastfeeding or the drug should be made by considering the importance of the drug for the mother.
Precautions & Warnings
Precautions include:
- Risk of low blood pressure, slow heart rate, and heart failure. Reduce or stop use if these occur.
- May worsen reactive airway diseases (e.g., asthma).
- Increases the effect of diabetes medications and may mask signs of low blood sugar (hypoglycemia).
- Risk of high blood pressure in people with untreated pheochromocytoma (a type of tumor).
- Stopping suddenly in people with heart disease may cause heart problems.
