Indications

Afalino 25 is used to treat partial-onset seizures in adult patients.

Pharmacology

Afalino 25 works by modulating the GABA-A receptor and blocking sodium channels. This helps to prevent seizures by raising the threshold for electrical signals and reducing the number of these signals in the brain. Its combined effects may provide better control of seizures.

Dosage & Administration

Adult patients: Afalino 25 can be taken with or without food, or as advised by a doctor.

Initial Dosage: Start with 12.5 mg once daily for the first two weeks.

Titration Regimen:

  • Week 3 and 4: 25 mg once daily
  • Week 5 and 6: 50 mg once daily
  • Week 7 and 8: 100 mg once daily
  • Week 9 and 10: 150 mg once daily
  • Week 11 and onward: 200 mg once daily

Maximum Dosage: If needed, the dose can be increased by 50 mg every two weeks, up to a maximum of 400 mg daily.

Interaction

When taking Afalino 25 with:

  • Phenytoin: Decrease the phenytoin dose by up to 50%.
  • Phenobarbital and Clobazam: Reduce their doses as necessary.
  • Lamotrigine, Carbamazepine: Increase their doses as necessary.
  • CYP2B6 and CYP3A Substrates: Increase their doses as needed.
  • CYP2C19 Substrates: Reduce their doses as needed.
  • Oral Contraceptives: Its effectiveness may decrease, so use additional or non-hormonal birth control methods.

Contraindications

Afalino 25 should not be used by patients who are allergic to Afalino 25 or any of its ingredients, or those with Familial Short QT syndrome.

Side Effects

Common side effects: Sleepiness, tiredness, dizziness, nausea, constipation, blurred vision, reduced appetite, diarrhea, or weakness.

Rare side effects: A very serious allergic reaction is rare.

Pregnancy & Lactation

There are no adequate studies in pregnant women, but Afalino 25 could harm the fetus. Afalino 25 should only be used during pregnancy if the potential benefits outweigh the risks. There is no information about its effects on breast milk or breastfeeding infants.

Precautions & Warnings

Afalino 25 may cause serious side effects like allergic reactions, QT shortening, and suicidal thoughts. Patients should not drive or operate heavy machinery during treatment until they understand how Afalino 25 affects them. Afalino 25 should be gradually stopped to avoid an increase in seizures.

Special considerations:

  • Drug Reaction with Eosinophilia and Systemic Symptoms (DRESS): Stop use if no other cause is found.
  • QT Shortening: Be cautious when used with other drugs that shorten the QT interval.
  • Suicidal Behavior and Ideation: Watch for signs of suicidal thoughts and behaviors.
  • Neurological Reactions: Be alert to tiredness and dizziness. Avoid driving or operating machinery until fully accustomed to the drug.
  • Withdrawal: Gradually reduce the dose to avoid worsening seizures.

Overdose Effects

If an overdose occurs, stop using Afalino 25 and provide supportive care until symptoms improve.

Therapeutic Class

Afalino 25 is an adjunct anti-epileptic drug.
