Indications
AFM-T is used for treating allergic eye conditions like conjunctivitis and keratitis, especially with severe swelling. AFM-T is also used for non-infectious inflammation of the front part of the eye, including conditions like anterior uveitis, episcleritis, and scleritis. Additionally, AFM-T helps with post-surgery irritation after strabismus, cataract, and glaucoma surgery, along with antimicrobial therapy.
Pharmacology
Fluorometholone works by inhibiting certain proteins that control inflammation. This helps reduce inflammation in the eye. Tetrahydrozoline, on the other hand, is an alpha agonist that tightens the blood vessels in the eye, helping to reduce redness.
Dosage & Administration
Adults: Instill 1 drop into the eye 2-3 times daily. For severe cases, the dose can be increased to 1 drop per hour for the first 1-2 days.
Children: Safety and effectiveness for children under 2 years have not been established.
Interaction
Tetrahydrozoline may interact with certain antidepressant drugs that affect the Monoamine Oxidase (MAO) enzyme, such as Isocarboxazid, Phenelzine, Toloxatone, and Tranylcypromine.
Contraindications
AFM-T is not recommended for use in infectious conjunctivitis or keratitis, eye injuries or ulcers, glaucoma, or diseases causing damage to the cornea or sclera. AFM-T should also be avoided in dry eye conditions.
Side Effects
Possible side effects include mild burning in the eye, temporary increase in eye pressure, redness, cataract, corneal issues, and glaucoma. Prolonged use may cause systemic effects.
Pregnancy & Lactation
There are no well-controlled studies on the use of Fluorometholone and Tetrahydrozoline during pregnancy or breastfeeding. AFM-T should only be used in pregnancy if the benefits outweigh the risks to the baby.
Precautions & Warnings
Prolonged use should be avoided. This medication should be used carefully in patients with severe blood circulation issues, metabolic disturbances, or those taking certain drugs like MAO inhibitors. AFM-T should also be used with caution in people with cataracts, herpes simplex infections, or dry nasal passages.
Overdose Effects
Overdose with Fluorometholone is unlikely to cause serious issues. However, if too much Tetrahydrozoline is taken, symptoms may include central nervous system, heart, and psychiatric problems, along with dilated pupils, cyanosis, and fever. Treatment may involve activated charcoal, gastric lavage, and medications to control symptoms. Vasopressors should be avoided.
Therapeutic Class
AFM-T is an ophthalmic steroid preparation combined with a decongestant.
Storage Conditions
Store the medication in a cool, dry place, away from heat and direct sunlight. Keep AFM-T out of the reach of children and do not use AFM-T more than 4 weeks after opening.
