Indications
Agorest 25 is used to treat major depression in adults, especially in people who don’t respond to or cannot tolerate selective serotonin reuptake inhibitors (SSRIs), and also helps prevent relapse.
Pharmacology
Agorest 25 is a melatonergic agonist that affects MT1 and MT2 receptors and an antagonist for 5-HT2C. Agorest 25 doesn’t affect monoamine uptake or interact with certain receptors like adrenergic, histaminergic, cholinergic, dopaminergic, or benzodiazepine. Agorest 25 helps adjust circadian rhythms in animal models and increases noradrenaline and dopamine in the frontal cortex without affecting serotonin levels.
Dosage & Administration
Adults: Take 25 mg once a day before bed for two weeks. If no improvement is seen, increase to 50 mg per day. Taking Agorest 25 at night is recommended as Agorest 25 improves sleep without causing drowsiness during the day.
Children under 18 years: Agorest 25 should only be used under a doctor’s advice. Agorest 25 is not recommended for children and teens since its safety and effectiveness have not been tested in this age group.
Elderly patients: Agorest 25 is safe for elderly patients (aged <75 years), but should not be used in patients over 75 since its effectiveness has not been proven in this age group.
Interaction
Agorest 25 is mainly processed by cytochrome P450 1A2 (CYP1A2) and partly by CYP2C9/19. Medications that affect these enzymes can change how well Agorest 25 works. Taking Agorest 25 with CYP1A2 inhibitors like fluvoxamine or ciprofloxacin is not recommended, as Agorest 25 can increase the amount of agomelatine in your body significantly.
Contraindications
Agorest 25 should not be used if you are allergic to any of its ingredients or if you have liver problems such as cirrhosis or active liver disease, or if your liver enzyme levels are more than three times the normal limit. Also, Agorest 25 should not be taken with strong CYP1A2 inhibitors like fluvoxamine or ciprofloxacin.
Side Effects
The most common side effects are headache, nausea, and diarrhea. Agorest 25 can also increase liver enzyme levels, so liver function should be monitored before starting treatment and every six weeks after that. Agorest 25 should not be used by people with liver problems. Agorest 25 has been shown to cause no significant sexual side effects compared to a placebo.
Pregnancy & Lactation
There are no clinical data on its use during pregnancy, and animal studies show no harm to the pregnancy or the baby. Agorest 25 should be used cautiously during pregnancy.
Agorest 25 is not known if Agorest 25 passes into breast milk. Since Agorest 25 is found in rat milk, breastfeeding should be stopped if treatment with Agorest 25 is necessary.
Precautions & Warnings
Agorest 25 should be used carefully in patients with risk factors for liver damage, and liver function should be checked before starting treatment. Agorest 25 should not be used in people over 75 or in elderly patients with dementia. Agorest 25 is not recommended for people under 18 years of age or those with a history of bipolar disorder, mania, or hypomania.
Overdose Effects
There is little experience with overdose. Some reports show that taking large amounts (up to 450 mg) causes drowsiness and stomach pain. There are no specific treatments for overdose, and treatment should focus on managing symptoms. Medical follow-up in a specialized setting is advised.
