Indications

Agoxin 0.25 is used to treat heart failure, atrial fibrillation with uncontrolled heart rate, acute left ventricular failure, and chronic left ventricular failure or congestive heart failure, especially when caused by high blood pressure, valve diseases (mainly mitral valve), or heart disease.

Pharmacology

Agoxin 0.25 is a cardiac glycoside used to manage atrial fibrillation and heart failure. Its main effects are increasing the strength of heart contractions (positive inotropic effect) and slowing down the heart’s electrical signals, especially through the atrioventricular node. Agoxin 0.25 also works directly on the blood vessels and indirectly through the nervous system, especially by increasing vagal (rest-and-digest) activity.

Dosage & Administration

By oral administration:

Rapid digitalization: 1-1.5 mg in divided doses over 24 hours.

Less urgent digitalization: 250-500 micrograms daily (higher doses may be divided).

Maintenance: 62.5-500 micrograms daily (higher doses may be divided), depending on kidney function and heart rate in atrial fibrillation.

Usual range: 125-250 micrograms daily (a lower dose may be appropriate for the elderly).

Interaction

Potassium-depleting diuretics increase the effects of digoxin. Calcium, especially when given rapidly through IV, can cause serious heart arrhythmias in patients using digoxin. Certain drugs such as quinidine, verapamil, amiodarone, propafenone, and others can increase digoxin levels in the blood. Some medications like antacids, kaolinpectin, sulfasalazine, and others may reduce how well digoxin is absorbed, lowering its levels in the blood.

Contraindications

Agoxin 0.25 is not recommended for use in patients with ventricular fibrillation or those who are allergic to digoxin or other digitalis medicines.

Side Effects

Side effects, usually from taking too much, include loss of appetite, nausea, vomiting, diarrhea, stomach pain, vision problems, headache, tiredness, confusion, mood changes, irregular heartbeats, and in rare cases, problems like breast enlargement in men with long-term use or low platelet count.

Pregnancy & Lactation

Agoxin 0.25 is passed into breast milk but at low levels that are not harmful to the baby.
