Indications
Aldactone is used to treat conditions such as heart failure, liver disease with fluid buildup, kidney problems, high aldosterone levels, high blood pressure, and low potassium levels caused by diuretics.
Pharmacology
Aldactone works by blocking the action of aldosterone, a hormone that retains sodium and removes potassium in the kidneys. This leads to increased water and sodium being removed from the body, while potassium is kept. Aldactone acts as both a diuretic and a medicine to lower blood pressure. Aldactone can be used alone or with other diuretics that act in other parts of the kidney.
Dosage & Administration
Edema in adults: Start with 100 mg per day, in one or two doses. This may be adjusted between 25 and 200 mg per day. Other diuretics may be added for faster results.
Primary hyperaldosteronism: For surgery preparation, take 100 to 400 mg per day. If surgery is not an option, Aldactone may be used long-term at the lowest effective dose.
Essential hypertension: For adults, start with 50 to 100 mg per day, in one or two doses.
Hypokalemia: Aldactone is useful for treating low potassium caused by diuretics, with a dosage ranging from 25 mg to 100 mg per day.
Interaction
Aldactone may cause high potassium levels when taken with ACE inhibitors. Combining Aldactone with alcohol, barbiturates, or narcotics can lead to low blood pressure when standing up. Using Aldactone with corticosteroids or ACTH can cause low potassium. Aldactone should not be taken with lithium, as Aldactone can increase the risk of lithium toxicity. Aldactone may also increase the effects of digoxin.
Contraindications
Aldactone is not recommended for patients with kidney problems, high potassium levels, or sensitivity to the drug.
Side Effects
Side effects may include breast enlargement in men, stomach issues like cramping and diarrhea, tiredness, headaches, skin rashes, confusion, problems with periods or no periods, and bleeding after menopause.
Pregnancy & Lactation
Aldactone should not be used during pregnancy. Canrenone, a product of the medicine, is found in breast milk. If necessary, alternative feeding methods for the baby should be used.
Precautions & Warnings
Patients should be monitored for fluid and electrolyte imbalances, especially potassium. High potassium can cause heart problems and be life-threatening, particularly in those with kidney issues or excessive potassium intake.
Overdose Effects
Overdose may cause drowsiness, confusion, dizziness, diarrhea, and vomiting. In case of overdose, vomiting or stomach lavage should be done.
Therapeutic Class
Potassium-sparing diuretics and Aldosterone antagonists.
Storage Conditions
Store in a cool, dry place, away from light. Keep out of reach of children.
