Indications

Aldonist is used for treating heart failure after a heart attack and for managing high blood pressure (hypertension).

Pharmacology

Aldonist works by blocking aldosterone from binding to receptors, helping reduce high blood pressure. Aldosterone normally increases blood pressure by making the body hold onto sodium. By blocking this action, Aldonist helps lower blood pressure and manage heart failure after a heart attack. Even though Aldonist raises certain blood levels, these changes do not undo the effects of the medicine.

Dosage & Administration

Heart failure after a heart attack: Start with 25 mg once daily, increasing to 50 mg once daily as tolerated within 4 weeks. Take with or without food.

High blood pressure: Start with 50 mg once daily. If this doesn’t lower the blood pressure enough, increase to 50 mg twice daily. Avoid higher doses because they don’t work better and may cause higher potassium levels.

Pediatric use: The safety and effectiveness for children are not known.

Geriatric use: No differences in safety or effectiveness were observed in older patients.

Interaction

Aldonist may increase the risk of high potassium levels when used with ACE inhibitors, angiotensin blockers, ciclosporin, tacrolimus, and trimethoprim. Aldonist may also reduce the blood pressure-lowering effect when taken with NSAIDs, glucocorticoids, or tetracosactide. Aldonist may increase the effects of blood pressure medications like α1-blockers, certain antidepressants, and other drugs. Some medications, like fluconazole or erythromycin, may increase the plasma level of this medicine.

Contraindications

Aldonist is not recommended for patients with high potassium levels, severe kidney or liver disease, or those using strong CYP3A4 inhibitors like Ketoconazole, Itraconazole, or Ritonavir. Aldonist should not be used with other potassium-sparing diuretics.

Side Effects

Common side effects include headache, dizziness, diarrhea, stomach pain, nausea, cough, or flu-like symptoms. Serious allergic reactions may include rash, itching, swelling, severe dizziness, or difficulty breathing.

Pregnancy & Lactation

Aldonist should only be used during pregnancy if the benefits outweigh the risks to the baby. Its effects on breastfeeding are unknown, so Aldonist should be decided whether to stop nursing or discontinue the medication, depending on the mother’s needs.

Precautions & Warnings

Aldonist should be used with caution in patients with high potassium levels, severe kidney disease, and in diabetic patients with heart failure after a heart attack, especially those with protein in their urine.
