Indications
Alfavir is used for treating chronic hepatitis B virus (HBV) infection in adults with stable liver disease.
Pharmacology
Alfavir is a prodrug of tenofovir, which enters liver cells and is converted into the active form. The active form, tenofovir diphosphate, stops HBV from replicating by preventing the virus from copying its DNA. Alfavir does not cause damage to mitochondria in cells.
Dosage & Administration
Prior to Starting Treatment: Patients should be tested for HIV-1 infection before starting Alfavir. Alfavir should not be used alone in patients with HIV-1 infection.
Adult Dosage: The recommended dosage is one 25 mg tablet once a day with food.
Renal Impairment: No dosage change is needed for patients with mild kidney issues or those on dialysis. If on dialysis, take Alfavir after dialysis.
Liver Impairment: No dosage change is needed for mild liver problems, but Alfavir is not recommended for patients with severe liver issues.
Pediatric Use: Alfavir is not recommended for children under 18.
Geriatric Use: No significant difference in safety or effectiveness has been observed in elderly patients.
Interaction
Alfavir is affected by drugs that change the activity of P-glycoprotein (P-gp) and BCRP, which could either reduce or increase its absorption. Alfavir should not be taken with drugs that reduce kidney function or compete for kidney processing, as this may increase side effects.
Contraindications
Alfavir is not recommended for patients with HIV-1 infection when used alone.
Side Effects
Common side effects include headache, stomach pain, tiredness, cough, nausea, and back pain. Serious side effects include liver issues, kidney problems, and a buildup of lactic acid in the blood.
Pregnancy & Lactation
If pregnant, consult a doctor before taking Alfavir. There is a slight risk of birth defects when used during pregnancy. Alfavir is not known if Alfavir passes into breast milk, so consult a doctor if breastfeeding.
Precautions & Warnings
Discontinuing Alfavir may lead to a worsening of hepatitis B. Alfavir should be used cautiously in patients with kidney problems and those taking other drugs that can affect the kidneys.
Overdose Effects
In case of overdose, supportive care is needed. Alfavir can be removed from the body through hemodialysis.
