Indications
Alkalina 600 is indicated for the management of acidosis in cardiac arrest, alkalinization of urine, and treatment of metabolic acidosis in conditions such as severe kidney disease, uncontrolled diabetes, circulatory problems from shock or dehydration, and severe primary lactic acidosis.
Pharmacology
Alkalina 600 is a systemic alkalizer that helps to increase plasma bicarbonate levels, balance excess hydrogen ions, and raise blood pH, which reverses symptoms of acidosis. Alkalina 600 also acts as a urinary alkalizer by raising the pH of urine and increasing the excretion of bicarbonate ions. Additionally, Alkalina 600 helps restore electrolyte balance lost in severe diarrhea.
Dosage & Administration
Oral: The usual dose is between 300 mg to 2 grams, taken 1 to 4 times daily.
Intravenous (Severe metabolic acidosis):
Adult: A slow injection of a hypertonic solution (up to 8.4%, 1000 mmol/L) or a continuous infusion of a weaker solution (usually 1.26%, 150 mmol/L). During advanced cardiac life support, 50 ml of an 8.4% solution may be given.
Elderly: Dosage adjustments may be needed.
Interaction
Alkalina 600 may interact with drugs like aspirin, memantine, and medications with enteric coatings. Alkalina 600 could also interfere with the effectiveness of certain drugs, such as ampicillin, atazanavir, ketoconazole, itraconazole, iron supplements, and sucralfate. Additionally, Alkalina 600 may have harmful interactions with corticosteroids, corticotropin, alkalis, calcium, diuretics, and potassium.
Contraindications
Alkalina 600 is contraindicated in people who have hypersensitivity, hypoventilatory states, excessive chloride loss due to vomiting or gastric suctioning, metabolic or respiratory alkalosis, hypocalcemia, and those using diuretics known to cause hypochloremic alkalosis.
Side Effects
Common side effects include headache, nausea, and irritability. If you experience muscle weakness, slow reflexes, confusion, swelling of the feet or ankles, black tarry stools, or coffee-ground vomit, notify your doctor immediately.
Pregnancy & Lactation
If you are pregnant, planning to become pregnant, or breastfeeding, use this only if the benefits outweigh the risks. Alkalina 600 is not recommended during the second and third trimesters due to potential renal effects and the possibility of raising blood pressure. This medication can pass into breast milk.
Precautions & Warnings
Caution is needed for those with heart disease, kidney disease, liver disease, high blood pressure, or any allergies because this product contains salt (sodium). Avoid using Alkalina 600 if on a salt-restricted diet.
Overdose Effects
In case of overdose, stop taking the bicarbonate and manage the alkalosis based on its severity. Intravenous sodium chloride (0.9%) can be given, and potassium chloride may be used if there’s low potassium. In severe cases of alkalosis, symptoms like hyperirritability or tetany can be treated with calcium gluconate, and an acidifying agent like ammonium chloride may be necessary.
