Indications

Alkenib 250 is used in combination with Capecitabine for treating advanced or metastatic breast cancer in patients whose tumors have high levels of HER2 and who have already been treated with an Anthracycline, a Taxane, and Trastuzumab. Treatment should only start after the disease has progressed on Trastuzumab.

Alkenib 250 is also used with Letrozole for treating postmenopausal women with hormone receptor-positive metastatic breast cancer that has high HER2 levels when hormonal therapy is needed. However, Alkenib 250 has not been compared to Trastuzumab in chemotherapy for metastatic breast cancer.

Pharmacology

Alkenib 250 is a kinase inhibitor that targets two specific receptors, EGFR (ErbB1) and HER2 (ErbB2). Alkenib 250 works by blocking tumor growth driven by these receptors. Studies have shown that Alkenib 250 works well when combined with other treatments like 5-FU. Alkenib 250 remains effective against cancer cells that become resistant to other treatments like Trastuzumab.

In hormone receptor-positive breast cancer, Alkenib 250 can help treat cells that have developed resistance to standard hormone therapies by targeting HER2 and EGFR receptors, which may become more active over time.

Alkenib 250 is absorbed variably when taken orally. After taking a dose, its peak plasma concentration is reached about 4 hours later. Taking Alkenib 250 with food increases the amount absorbed into the bloodstream.

Dosage & Administration

For HER2-Positive Metastatic Breast Cancer: The recommended dose is 1,250 mg once daily with Capecitabine (2,000 mg/m2/day) for 21-day cycles. Lapatinib should be taken at least one hour before or after a meal.

For Hormone Receptor-Positive, HER2-Positive Metastatic Breast Cancer: The recommended dose is 1,500 mg once daily with Letrozole (2.5 mg). Lapatinib should be taken at least one hour before or after a meal.

Pediatric Use: The safety of this medicine in children has not been established.

Interaction

Alkenib 250 can affect the metabolism of other drugs, especially those processed by CYP3A4 and P-glycoprotein. Caution should be taken when used with other medications that are processed by these enzymes. For example, taking Alkenib 250 with Ketoconazole increases its levels, while Carbamazepine decreases its levels.

Alkenib 250 may also interact with medications like Digoxin, increasing its effects, so monitoring is needed if used together.

Contraindications

Alkenib 250 is not recommended for patients with a known severe allergic reaction to this drug or any of its components.

Side Effects

Common side effects include diarrhea, nausea, rash, and fatigue. These can occur during treatment with either Capecitabine or Letrozole. Serious side effects are rare but may include severe diarrhea and liver issues.

Pregnancy & Lactation

If pregnant, planning to become pregnant, or breastfeeding, consult your doctor before using this medication. Alkenib 250 can harm the fetus, and there is no data on its effects while breastfeeding. Women should avoid breastfeeding during treatment and for one week after stopping Alkenib 250.

Precautions & Warnings

Alkenib 250 may cause a decrease in heart function, so heart monitoring is recommended. Liver problems, including severe liver damage, can also occur, so liver function should be checked regularly during treatment. Patients with liver problems should avoid using Alkenib 250 or may need a dose reduction.

Diarrhea is another possible side effect. Alkenib 250 is important to manage Alkenib 250 early with medications like Loperamide. Severe cases may require additional treatment or stopping the medicine.

Overdose Effects

Overdose symptoms can include nausea, vomiting, diarrhea, dizziness, and heart issues. Seek immediate medical attention if overdose is suspected.
