Indications
Amaryl M 1 is used alongside diet and exercise to help control blood sugar levels in people with type 2 diabetes. Amaryl M 1 is used when treatment with just glimepiride or metformin alone doesn’t provide enough control. Amaryl M 1 can also replace the combination therapy of glimepiride and metformin.
Pharmacology
Glimepiride is an antidiabetic medicine that helps lower blood sugar. Amaryl M 1 works by encouraging the release of insulin from the pancreas, which helps manage blood sugar. Amaryl M 1 also improves how sensitive the pancreas is to glucose, making insulin secretion easier. Amaryl M 1 can also reduce glucose production in the liver and help the body use insulin better. Glimepiride’s effect lasts for 24 hours in non-fasting diabetics.
Metformin is another medication used for type 2 diabetes. Amaryl M 1 lowers blood sugar before and after meals. Amaryl M 1 works by decreasing the amount of sugar the liver produces, reducing sugar absorption in the intestines, and improving the body’s ability to use insulin. Unlike other medications, Amaryl M 1 does not cause low blood sugar (hypoglycemia).
Dosage & Administration
Adults: Take the lowest dose that works for your blood sugar control. Blood sugar levels should be checked regularly during treatment. If you miss a dose, don’t take a larger dose to make up for Amaryl M 1.
As diabetes control improves, you may need a lower dose of glimepiride. Talk to your doctor about reducing or stopping the medication to avoid low blood sugar. The highest recommended daily dose is 8 mg of glimepiride and 2000 mg of metformin.
Switching from separate tablets: If you’re switching from taking glimepiride and metformin as separate tablets, this combination tablet should be taken based on the current doses you’re using.
Interaction
For Glimepiride: Glimepiride is processed by the liver, so be careful when taking Amaryl M 1 with medications that affect the liver (such as rifampicin or fluconazole). Some medicines, like insulin, may increase the blood sugar-lowering effect, while others, like corticosteroids, may weaken Amaryl M 1. Alcohol and some other medications may also affect how well glimepiride works.
For Metformin: Avoid alcohol because Amaryl M 1 increases the risk of a serious condition called lactic acidosis. Be careful with contrast agents used in imaging procedures—metformin should be stopped 48 hours before and after using them. Certain drugs like glucocorticoids and diuretics can raise blood sugar, while ACE inhibitors can lower Amaryl M 1. Always check your blood sugar more often if you’re taking any of these medications.
Contraindications
For Glimepiride: Amaryl M 1 is not for people allergic to glimepiride, metformin, or other similar drugs. Amaryl M 1 should also be avoided during pregnancy and breastfeeding. Amaryl M 1 is not recommended for people with severe liver issues or on dialysis.
For Metformin: Amaryl M 1 is not for people allergic to metformin, or those with severe kidney problems, certain heart or lung conditions, or liver disease. People with alcohol use disorder should also avoid Amaryl M 1.
Side Effects
For Glimepiride: Some people may experience low blood sugar (hypoglycemia), which can cause symptoms similar to a stroke. Other side effects include nausea, abdominal pain, or liver problems. Rarely, people may experience allergic reactions or changes in their blood counts.
For Metformin: The most common side effects are nausea, vomiting, diarrhea, and stomach pain, especially when starting the medication. These symptoms usually improve over time. Very rarely, metformin can cause lactic acidosis, a serious condition. Long-term use may reduce vitamin B12 levels, which can cause nerve problems in some cases.
Pregnancy & Lactation
For Glimepiride: Amaryl M 1 should not be used during pregnancy. Pregnant women should switch to insulin therapy. If you’re planning a pregnancy, talk to your doctor about changing your medication.
For Metformin: During pregnancy, metformin should not be used. Insulin should be used instead to control blood sugar. Metformin may pass into breast milk, so Amaryl M 1’s better not to take Amaryl M 1 while breastfeeding.
Precautions & Warnings
For Glimepiride: There is a higher risk of low blood sugar in the first few weeks of treatment. If you have any risk factors for hypoglycemia, your dose may need to be adjusted. Regular monitoring is important. If you have G6PD deficiency, use this medication with caution.
For Metformin: Amaryl M 1 is recommended to regularly check thyroid hormone levels while on metformin. Long-term use may lower vitamin B12 levels, so monitoring those levels is important.
Overdose Effects
For Glimepiride: Taking too much glimepiride can cause dangerously low blood sugar. If an overdose is suspected, immediately seek medical help. You may need to take sugar or glucose. Severe overdoses may require hospitalization and intravenous treatment.
