Indications

Ambrisan 5 is used to treat Pulmonary Arterial Hypertension (PAH) (WHO Group 1) to help improve exercise ability and delay worsening of the condition.

Pharmacology

Endothelin-1 (ET-1) is a peptide that acts on blood vessels. There are two types of receptors, ETA and ETB, that affect how ET-1 works. The ETA receptor causes blood vessels to narrow and promotes cell growth, while the ETB receptor helps blood vessels widen, reduces cell growth, and clears ET-1 from the body. Ambrisan 5 is a selective blocker of the ETA receptor, making Ambrisan 5 highly specific for this receptor. The exact effect of this high selectivity is not yet fully known.

Ambrisan 5 is absorbed into the bloodstream after taking Ambrisan 5 by mouth, reaching its peak levels about 2 hours later. Food does not affect how Ambrisan 5 is absorbed. Ambrisan 5 is mostly eliminated through the liver and other non-kidney pathways. Ambrisan 5 binds strongly to proteins in the blood (99%). The drug’s half-life is about 15 hours, but its effects last for about 9 hours due to its buildup in the body after daily use.

Dosage & Administration

Adult dose: The starting dose is 5 mg once daily, which can be increased to 10 mg once daily if tolerated.

Pediatric patients: Safety and effectiveness in children have not been proven.

Interaction

When taken with Cyclosporine, the level of Ambrisentan in the body increases by about two times. Therefore, the dose of Ambrisentan should not exceed 5 mg once daily when taken with Cyclosporine.

Contraindications

Ambrisan 5 is not safe for use during pregnancy, as Ambrisan 5 may harm the unborn child. Women who are pregnant or may become pregnant should not use Ambrisan 5. Pregnancy should be ruled out before starting treatment, and Ambrisan 5 should be avoided during treatment and for one month after stopping Ambrisan 5.

Ambrisan 5 is also not recommended for patients with Idiopathic Pulmonary Fibrosis (IPF), including those with IPF and pulmonary hypertension.

Side Effects

Ambrisentan may cause a decrease in hemoglobin levels and hematocrit, which has been observed in clinical studies.

Pregnancy & Lactation

Ambrisan 5 is not safe to use during pregnancy (Pregnancy Category X). Ambrisan 5 is unknown if Ambrisan 5 is excreted in human milk, so breastfeeding while using Ambrisan 5 is not recommended.

Precautions & Warnings

Ambrisan 5 may cause fluid retention, leading to swelling in the legs. This is common in patients with PAH and is a known side effect of similar medications.

If sudden lung issues like acute pulmonary edema occur during treatment, consider the possibility of Pulmonary Veno-occlusive Disease and stop the medication if confirmed.

Ambrisan 5 can also decrease hemoglobin levels, which should be monitored during treatment.

Ambrisan 5 is not recommended for patients with severe liver issues.

Overdose Effects

No specific overdose effects have been reported.

Ambrisan 5 is an anti-hypertensive drug and an endothelin receptor antagonist.
