Indications
Aminophylline is used to treat and prevent bronchospasm caused by asthma, emphysema, and chronic bronchitis. Aminophylline is also used in adults to treat cardiac asthma and heart failure.
Pharmacology
Aminophylline is a combination of theophylline and ethylenediamine. Ethylenediamine is not active but helps theophylline dissolve in water. Theophylline works by relaxing the muscles in the airways. This is thought to happen through several mechanisms, including increasing a substance called cAMP inside cells, blocking adenosine receptors, affecting prostaglandins, and impacting calcium inside cells.
Dosage & Administration
Oral:
Chronic bronchospasm:
Adults: Initially, 225-450 mg twice a day, increased if needed.
Children (over 3 years old): Initially, 12 mg/kg per day, increased to 24 mg/kg per day after one week.
Intravenous:
Acute severe bronchospasm:
Adults: Initial dose: 5 mg/kg (based on ideal body weight) or 250-500 mg by slow injection or infusion over 20-30 minutes. Maintenance dose: 0.5 mg/kg per hour. Maximum rate: 25 mg per minute.
Children: Initial dose: same as adults. Maintenance dose: 1 mg/kg per hour for children 6 months-9 years, and 0.8 mg/kg per hour for children 10-16 years.
Elderly: Dose may need to be reduced.
Hepatic impairment: Dose may need to be reduced.

Contraindications
Aminophylline should not be given to patients who are allergic to xanthines or ethylenediamine. Aminophylline should also be avoided in patients with active peptic ulcers, as Aminophylline may increase stomach acid.
Side Effects
Common side effects include stomach irritation, nausea, vomiting, diarrhea, stomach pain, and shaking. High doses can cause heart rhythm problems or seizures. Other possible side effects include headache, restlessness, difficulty sleeping, muscle twitching, increased heart rate, low blood pressure, and changes in urination. Some patients may also experience inappropriate ADH syndrome.
Pregnancy & Lactation
If pregnant or breastfeeding, consult a doctor before using Aminophylline. Aminophylline should only be used during pregnancy if the benefits outweigh the risks, especially in the first trimester.
Precautions & Warnings
Aminophylline should be used carefully in patients with peptic ulcers, hyperthyroidism, high blood pressure, heart problems, or epilepsy. Aminophylline should also be used with caution in people with heart failure, liver problems, chronic alcoholism, acute fever, or in newborns and elderly patients, as Aminophylline may increase the level of the drug in the blood.
