Indications
Apiban 2.5 is used to reduce the risk of stroke and blood clots in patients with a certain type of irregular heartbeat (nonvalvular atrial fibrillation). Apiban 2.5 is also used to prevent deep vein thrombosis (DVT) that can cause lung problems in patients who have had hip or knee replacement surgery. Additionally, Apiban 2.5 helps treat DVT and pulmonary embolism (PE), and lowers the risk of these problems returning after initial treatment.
Pharmacology
Apiban 2.5 works by blocking a protein (factor Xa) in the blood that is involved in clotting. By doing this, Apiban 2.5 prevents blood clots from forming. Apiban 2.5 can affect clotting tests such as prothrombin time (PT) and INR, but these tests are not very helpful in monitoring the effects of this medicine.
Dosage & Administration
Recommended Dose: The usual dose is 5 mg taken twice a day by mouth.
Dosage Adjustments: For patients with certain conditions (age ≥80, weight ≤60 kg, or high serum creatinine), the dose should be reduced to 2.5 mg twice daily.
Drug Interactions: If taken with medicines that strongly affect the enzymes CYP3A4 and P-gp (e.g. ketoconazole, ritonavir), the dose should be 2.5 mg twice daily.
Missed Dose: If a dose is missed, take Apiban 2.5 as soon as possible on the same day, but do not take two doses to make up for the missed one.
Discontinuation for Surgery: Stop taking Apiban 2.5 48 hours before surgery with a high risk of bleeding or 24 hours before surgery with a low risk of bleeding.
Switching from Warfarin to Apixaban: Stop warfarin when the INR is below 2.0 and start Apixaban.
Switching from Apixaban to Warfarin: Stop Apixaban and start both a parenteral anticoagulant and warfarin at the next dose time.
Switching Between Anticoagulants: Stop one and start the other at the next scheduled dose.
Interaction
Apiban 2.5 is affected by drugs that alter the activity of CYP3A4 and P-gp. Medications that increase these enzymes may raise the risk of bleeding, while those that reduce their activity may increase the risk of stroke.
Contraindications
Apiban 2.5 should not be used in patients with active bleeding or severe allergic reactions to Apiban 2.5 (such as anaphylaxis).
Side Effects
Apiban 2.5 may cause skin rashes or severe allergic reactions.
Pregnancy & Lactation
If pregnant or breastfeeding, talk to your doctor before using Apiban 2.5. Apiban 2.5 may increase the risk of bleeding during pregnancy and delivery. Apiban 2.5 is not known whether Apiban 2.5 passes into breast milk, so you may need to stop breastfeeding while taking Apiban 2.5.
Precautions & Warnings
Stopping Apiban 2.5 without replacing Apiban 2.5 with another anticoagulant can increase the risk of blood clots. If Apiban 2.5 needs to be stopped for a reason other than bleeding, another anticoagulant should be considered.
Overdose Effects
Overdose can increase the risk of bleeding. There is no antidote, but activated charcoal may help if the overdose was recent.
