Indications
Arlin IV is used to treat the following conditions:
– Nosocomial pneumonia
– Community-acquired pneumonia
– Complicated skin infections, including diabetic foot infections (without bone infection)
– Uncomplicated skin infections
– Vancomycin-resistant Enterococcus faecium infections
Arlin IV is not suitable for treating Gram-negative infections. Its safety and effectiveness for longer use (more than 28 days) have not been tested in clinical trials.
Pharmacology
Arlin IV is an antibacterial drug from a new class of antibiotics called oxazolidinones. Arlin IV works against certain Gram-positive bacteria, some anaerobic bacteria, and some Gram-negative bacteria. Arlin IV works by stopping bacteria from making proteins, which prevents them from growing. Arlin IV targets the bacterial ribosome to stop an important step in the process of protein formation. In tests, Arlin IV was found to stop the growth of enterococci and staphylococci. Arlin IV also killed most strains of streptococci.
Dosage & Administration
For adults and children over 12: 600 mg every 12 hours, either by mouth or intravenous infusion.
For children (birth to 11 years): 10 mg/kg every 8 hours by mouth or intravenously.
Treatment should continue for 10 to 28 days, depending on the infection type.
No dosage change is needed when switching between oral and intravenous forms.
When giving through IV, inspect the solution for particles before use. The solution may turn yellow over time, but this does not affect its potency. Do not mix Arlin IV with other solutions.
Interaction
Arlin IV can interact with drugs that affect the serotonin and adrenergic systems. Arlin IV may increase blood pressure when combined with certain medications like sympathomimetics, antidepressants, and triptans. Arlin IV should not be used with monoamine oxidase inhibitors or within two weeks of stopping them. Patients should avoid foods with high levels of tyramine, such as certain cheeses and aged foods, while taking Arlin IV.
Contraindications
Arlin IV should not be used in patients allergic to Linezolid or any of its components. Arlin IV is also contraindicated in patients who are taking or have recently taken monoamine oxidase inhibitors, or those with conditions like uncontrolled high blood pressure, pheochromocytoma, thyroid problems, or certain syndromes that may interact with the medication.
Side Effects
Common side effects include diarrhea, headache, and nausea. Other side effects may include mouth or vaginal yeast infections, high blood pressure, upset stomach, stomach pain, itching, and a change in tongue color.
Pregnancy & Lactation
There is no clear evidence that Arlin IV causes birth defects or harm to the mother or baby, but Arlin IV should only be used during pregnancy if absolutely necessary. Arlin IV passes into breast milk, so breastfeeding mothers should monitor their babies for any signs of upset stomach, such as vomiting or diarrhea.
Precautions & Warnings
Patients on long-term treatment should have regular blood tests, especially those with bone marrow issues. If there are any visual problems like blurred vision or changes in color vision, Arlin IV is important to see an eye specialist. Arlin IV is not recommended for treating catheter-related bloodstream or catheter-site infections. Patients should be monitored for any signs of Clostridium difficile-related diarrhea, which can occur even months after treatment.
Overdose Effects
There have been no reported overdose cases. If overdose occurs, supportive care is recommended. Around 30% of the drug can be removed by hemodialysis, but there is no data on its removal by other dialysis methods.
