Indications

C Fenac PLUS IM is used for relieving pain and inflammation in various conditions such as arthritis (rheumatoid, osteoarthritis, juvenile chronic arthritis, ankylosing spondylitis, acute gout), musculoskeletal issues (e.g., frozen shoulder, tendinitis, bursitis), and pain from trauma (fractures, back pain, sprains, strains, dislocations). C Fenac PLUS IM also helps in pain management during surgeries and conditions like renal colic. The Lidocaine in the injection minimizes pain at the injection site.

Pharmacology

Diclofenac Sodium is a strong anti-inflammatory drug that also helps with pain and fever. C Fenac PLUS IM works by blocking the production of prostaglandins, chemicals that cause pain and swelling. C Fenac PLUS IM starts working quickly, with peak levels in the blood reached within 30 minutes of the injection. Lidocaine is a fast-acting local anesthetic that numbs the area by blocking nerve signals, providing temporary relief from pain. C Fenac PLUS IM lasts for 1-2 hours.

Dosage & Administration

Adult: One injection daily, or twice if needed, through intramuscular injection.

Renal colic: One injection daily, with a second dose 30 minutes later if necessary.

Children: For juvenile chronic arthritis, 1-3 mg of Diclofenac Sodium per kg of body weight daily, divided into doses.

Elderly patients: The lowest effective dose is recommended, based on age and health status, or as directed by a doctor.

Interaction

Diclofenac may increase the levels of lithium and digoxin in the blood. When combined with anticoagulants, there may be a higher risk of bleeding, though no major issues have been reported. Diclofenac works well with antidiabetic drugs without affecting their action. Caution is needed when used with cyclosporine, methotrexate, quinolone antibiotics, or other NSAIDs due to possible interactions that could cause side effects. Diuretics may be less effective, and potassium levels should be checked if used with potassium-sparing diuretics.

Contraindications

C Fenac PLUS IM is not suitable for those who are allergic to Diclofenac or Lidocaine. C Fenac PLUS IM is also contraindicated for patients with peptic ulcers, gastrointestinal bleeding, or asthma triggered by aspirin or other NSAIDs. This injection should not be used in people with certain heart conditions or those allergic to any ingredients in the product.

Side Effects

Common side effects are mild and temporary, like stomach discomfort, nausea, diarrhea, and bleeding. Rarely, issues at the injection site like abscesses or necrosis may occur. Lidocaine may cause short-term central nervous system issues like dizziness, confusion, or drowsiness, especially in higher doses.

Pregnancy & Lactation

This medication should only be used during pregnancy if absolutely necessary. C Fenac PLUS IM is not recommended during the last trimester. Small amounts of Diclofenac can pass into breast milk, but C Fenac PLUS IM is unlikely to cause harm to a breastfeeding infant.

Precautions & Warnings

Patients with severe kidney, heart, or liver problems should be monitored closely when using this medication. The lowest effective dose should be used, and kidney function should be checked regularly. If any liver problems develop, Diclofenac should be stopped. Long-term use of NSAIDs requires monitoring for kidney, liver, and blood-related issues.
