Indications

Citofer 210 is used to control high phosphorus levels in the blood for adult patients with chronic kidney disease who are on dialysis. Citofer 210 is also used to treat iron deficiency anemia in adult patients with chronic kidney disease who are not on dialysis.

Pharmacology

Citofer 210 works by binding to phosphate in the digestive system and turning into ferric phosphate, which is insoluble and leaves the body through stool. This process lowers phosphate levels in the blood. In the digestive system, Citofer 210 is changed into a different form of iron, which is absorbed into the blood and carried by a protein called transferrin, helping to form hemoglobin in red blood cells.

Dosage & Administration

For Chronic Kidney Disease on Dialysis: The starting dose is 2 tablets, taken three times a day with meals. The dose can be adjusted by 1 to 2 tablets as needed to keep the phosphorus levels at target, up to a maximum of 12 tablets a day. The dose can be increased every week or longer if necessary.

For Iron Deficiency Anemia in Chronic Kidney Disease not on Dialysis: The starting dose is 1 tablet, taken three times a day with meals. The dose can be adjusted as needed to reach and maintain the hemoglobin goal, up to a maximum of 12 tablets a day, or as advised by a doctor.

Use in Children & Adolescents: The safety and effectiveness of Citofer 210 have not been established for children and adolescents.

Interaction

Oral drugs like Doxycycline and Ciprofloxacin should not be taken together with Citofer 210. There should be a gap between taking these medications.

Contraindications

Citofer 210 should not be used by patients with iron overload syndrome, such as Hemochromatosis.

Side Effects

Common side effects include discolored stools, diarrhea, constipation, nausea, vomiting, cough, stomach pain, and high potassium levels in the blood.

Pregnancy & Lactation

There is not enough data on the use of Citofer 210 in pregnant women or breastfeeding mothers. Consult a doctor before using Citofer 210 during pregnancy or lactation.

Precautions & Warnings

Iron absorption from Citofer 210 can lead to high iron levels in the body. This may increase serum ferritin and transferrin saturation (TSAT) levels, requiring a dose reduction or stopping intravenous iron. Keep Citofer 210 out of reach of children as accidental overdose can be harmful. If overdose occurs, immediately contact a doctor or poison control.

Overdose Effects

There is no specific data on overdose of Citofer 210. In patients with chronic kidney disease, the maximum dose studied was 2,520 mg of ferric iron per day.
