Indications
Comet 1gm is used to treat type 2 diabetes, especially in overweight patients when diet and exercise alone do not provide enough control of blood sugar. In adults, Comet 1gm can be used alone or with other diabetes medicines or insulin. In children aged 10 and older, Comet 1gm can also be used alone or with insulin. Comet 1gm has been shown to help prevent complications from diabetes in overweight adults when diet alone has not worked.
Pharmacology
Comet 1gm is an oral drug used to manage type 2 diabetes. Comet 1gm helps lower blood sugar levels both before and after meals. Comet 1gm works differently from other diabetes medicines and does not cause low blood sugar. Comet 1gm lowers sugar production in the liver, reduces sugar absorption in the intestines, and helps the body use insulin better.
Dosage & Administration
Adults: The usual starting dose is 500 mg twice a day or 850 mg once a day, taken with meals. The dose may be increased by 500 mg each week or 850 mg every two weeks, up to a maximum of 2000 mg per day, divided into multiple doses. In some cases, a maximum of 2550 mg per day can be used. For better tolerance, doses above 2000 mg may be divided into three doses with meals.
Children: The usual starting dose is 500 mg twice a day with meals. The dose can be increased by 500 mg weekly up to a maximum of 2000 mg per day, divided into multiple doses.
Extended Release Tablets: The usual starting dose is 500 mg once a day with the evening meal. The dose can be increased weekly by 500 mg, up to a maximum of 2000 mg once a day or 1000 mg twice a day. Extended release tablets should be swallowed whole and should not be crushed, cut, or chewed.
Renal Impairment: Do not use if kidney function is severely reduced. Assess the benefits and risks if kidney function declines.
Interaction
No known interactions with Furosemide, but Nifedipine may increase its absorption. Comet 1gm can interact with other cationic drugs eliminated by the kidneys, such as Amiloride, Digoxin, and Morphine. Some drugs can raise blood sugar and affect diabetes control, including diuretics, corticosteroids, and certain oral contraceptives.
Contraindications
Comet 1gm should not be used in patients who are allergic to any of its ingredients or have certain conditions like acute metabolic acidosis, severe kidney failure, or dehydration. Comet 1gm is also not recommended for people with certain diseases that can lower oxygen levels in the body, like heart or lung failure, or recent heart attacks.
Side Effects
Common side effects include gastrointestinal issues like nausea, vomiting, diarrhea, and stomach pain, especially when starting treatment. These usually go away on their own. Comet 1gm can also cause taste changes, low vitamin B12 levels, and in rare cases, liver problems or skin reactions like itching. If any severe reactions occur, contact a doctor immediately.
Pregnancy & Lactation
Comet 1gm is not recommended during pregnancy. If you are pregnant or planning to become pregnant, insulin should be used instead of this medicine to control blood sugar. Comet 1gm is excreted in breast milk, so breastfeeding is not recommended while taking this medicine due to limited data on its safety in infants.
Precautions & Warnings
Comet 1gm can accumulate in the body if kidney function is impaired, leading to a serious condition called lactic acidosis. Regular kidney function checks are advised. Comet 1gm can also lower vitamin B12 levels and cause low blood sugar when used with insulin. Make sure to stop using Comet 1gm before surgery or tests that could affect kidney function. Avoid using Comet 1gm if you are dehydrated or have a serious infection.
Overdose Effects
Taking too much can cause lactic acidosis, a medical emergency. Symptoms may include muscle pain, weakness, and difficulty breathing. Hemodialysis is used to remove excess Metformin from the body.
