Indications
Corabid 5 is used to treat chronic stable angina pectoris in patients with coronary artery disease who have a normal heart rhythm. Corabid 5 is especially for those who cannot tolerate beta-blockers or when beta-blockers are not effective enough in controlling symptoms. Corabid 5 can also be used alongside beta-blockers in patients who are not fully controlled by beta-blockers and have a heart rate greater than 60 beats per minute.
Pharmacology
Corabid 5 is a medication that lowers the heart rate. Corabid 5 works by blocking a specific current in the heart that controls how the heart beats. By slowing the heart rate, Corabid 5 reduces the heart’s workload and oxygen demand. This also improves blood flow to the heart by extending the relaxation phase of the heart. Corabid 5 specifically targets the sinus node without affecting other parts of the heart, such as the atria or ventricles, or the heart’s ability to contract.
Dosage & Administration
Adult: The typical starting dose is 5 mg twice a day, which can be increased to 7.5 mg twice a day after 3-4 weeks if needed. The usual dose is one tablet in the morning and one in the evening with meals. If the heart rate drops below 50 beats per minute at rest or if symptoms like dizziness occur, the dose should be reduced to 2.5 mg twice a day.
Elderly: For patients over 75 years, a lower starting dose of 2.5 mg twice a day is recommended. The dose can be increased if necessary after evaluation.
Renal insufficiency: No change in dose is needed for patients with mild kidney problems. However, Corabid 5 should be used cautiously for those with severe kidney issues.
Hepatic impairment: No adjustment is needed for patients with mild liver problems, but caution is advised for those with moderate liver problems. Corabid 5 should not be used in patients with severe liver issues.
Children and adolescents: Corabid 5 is not recommended for children or adolescents because there is not enough information on its safety and effectiveness in this age group.
Interaction
Corabid 5 should not be used with medications that prolong the QT interval in the heart, as this could make the problem worse. This includes certain cardiovascular medications like quinidine, sotalol, and amiodarone, as well as non-cardiovascular drugs like pimozide and certain antibiotics. If these combinations are necessary, close monitoring of the heart is required.
Contraindications
Corabid 5 should not be used in patients with a history of allergic reactions to Corabid 5, or in those with a resting heart rate below 60 beats per minute before treatment, heart failure, severe hypotension, severe liver problems, or certain heart conditions like sick sinus syndrome, sino-atrial block, or 3rd degree AV block. Corabid 5 is also not recommended when combined with strong cytochrome P-450 3A4 inhibitors such as certain antifungals, antibiotics, or HIV medications.
Side Effects
Possible side effects include visual disturbances, blurred vision, slow heart rate, first-degree AV block, irregular heartbeats, headaches, and dizziness.
Pregnancy & Lactation
Corabid 5 is not recommended during pregnancy as studies in animals have shown potential risks. Corabid 5 can also pass into breast milk, so Corabid 5 should not be used while breastfeeding.
Precautions & Warnings
Special care should be taken in patients with low blood pressure, heart arrhythmias, or those taking medications that affect the QT interval. Corabid 5 should also be used with caution in patients with moderate liver or severe kidney problems.
Overdose Effects
No specific overdose effects have been noted. However, overdose may lead to severe symptoms and should be treated as per general overdose management protocols.
