Indications
Cordarone is used to treat serious heart rhythm problems such as ventricular fibrillation, ventricular tachycardia, atrial fibrillation, and atrial flutter. Cordarone is also used to start treatment and prevent frequent ventricular fibrillation and unstable ventricular tachycardia in patients who do not respond to other treatments.
Pharmacology
Cordarone is a medicine that helps control abnormal heart rhythms. Cordarone works by slowing down the heart’s electrical system, making Cordarone more stable. This helps the heart beat normally. Cordarone can also relax blood vessels, which may help patients with heart failure, although Cordarone can cause low blood pressure in some cases.
Dosage & Administration
Tablet: 200 mg three times a day for one week, then reduce to 200 mg twice a day or as needed to control arrhythmia. Cordarone should be taken with meals to prevent stomach upset.
Injection: Start with 1000 mg over the first 24 hours. The infusion should be done in the following way: 150 mg per 100 mL over 10 minutes, then 1 mg/min for 6 hours, and 0.5 mg/min after that. If there are sudden episodes of ventricular fibrillation or unstable tachycardia, the starting dose may be repeated, and the infusion rate may be increased.
Interaction
Cordarone may interact with medications such as beta-blockers (like Atenolol), calcium channel blockers (like Verapamil), and other drugs like Digoxin, Flecainide, and Warfarin, which can cause slow heart rate or other issues. Cordarone can also increase the blood levels of some drugs, so the dosage of those drugs may need to be adjusted.
Contraindications
Cordarone should not be used in patients with severe heart problems, like heart shock, certain heart rhythm issues, or if there has been fainting due to slow heart rate. Cordarone is also not suitable for people allergic to the drug or its components.
Side Effects
The most serious side effects are related to the lungs, which can sometimes be fatal. Patients should report any symptoms like cough, fever, or painful breathing. Other side effects can include liver problems, eye issues, numbness, slow heart rate, skin changes, thyroid problems, or digestive issues. If these occur, the dose may need to be adjusted or the treatment stopped.
Pregnancy & Lactation
Cordarone is not recommended for use during pregnancy because Cordarone may affect the thyroid of the baby. If necessary, the patient should be informed about the potential risks. The safety of using Cordarone during breastfeeding has not been established.
Precautions & Warnings
Care should be taken in patients with iron overload or certain types of anemia. If the treatment does not work as expected, other causes of the condition should be investigated.
