Indications
Cortef IV/IM is used for treating conditions like:
- Adrenal insufficiency (both primary and secondary)
- Acute adrenal insufficiency
- Shock that doesn’t respond to regular treatments
- Congenital adrenal hyperplasia
- High calcium levels caused by cancer
- Thyroiditis (inflammation of the thyroid)
- Rheumatic disorders
- Skin diseases like severe seborrheic dermatitis, severe psoriasis, pemphigus, and erythema multiforme
- Severe allergic conditions like asthma, dermatitis, and hay fever
- Eye diseases such as herpes zoster ophthalmicus and iritis
- Some gastrointestinal and pulmonary diseases
- Blood disorders like autoimmune anemia and thrombocytopenia
- Certain types of cancer like leukemia and lymphoma
- Conditions with swelling (edematous states)
- Exacerbations of multiple sclerosis
Pharmacology
Cortef IV/IM works by blocking the enzyme that helps make prostaglandins, which are involved in inflammation. This reduces inflammation and controls the production of certain proteins in the body.
Dosage & Administration
Tablet: The dosage can range from 20 mg to 240 mg per day, depending on the severity of the condition. For less severe cases, lower doses are used, while more serious cases may need higher doses. The dosage should be adjusted to find the amount that works best. After a good response is seen, the dose should be gradually reduced to the lowest amount that keeps the condition under control.
Injection: For adults, the initial dose can be between 100 mg to 500 mg, depending on the severity. For children, the dose varies with age:
- Up to 1 year: 25 mg
- 1-5 years: 50 mg
- 6-12 years: 100 mg
Interaction
Some drugs like Phenobarbital, Phenytoin, and Rifampin may lower the effect of this medicine. On the other hand, drugs like Troleandomycin and Ketoconazole may increase its effects. The dose may need adjustment to avoid side effects.
Contraindications
Cortef IV/IM should not be used by people with severe fungal infections or known allergies to any part of this medicine.
Side Effects
Cortef IV/IM is generally well tolerated, but high doses for long periods may cause:
- Heart problems
- Increased hunger and weight gain
- Upset stomach
- Skin issues like acne
- Changes in mood, including feeling very happy or depressed
