Indications
Cosec MUPS 20 is used to treat:
- Gastric and duodenal ulcers
- Ulcers caused by NSAIDs (Non-Steroidal Anti-Inflammatory Drugs)
- Prevent ulcers in people with a history of NSAID-related ulcers
- Gastro-esophageal reflux disease (GERD)
- Long-term management of acid reflux
- Acid-related indigestion
- Severe reflux esophagitis
- Prevention of acid aspiration during surgery
- Zollinger-Ellison syndrome
- Helicobacter pylori-related stomach ulcers
Pharmacology
Cosec MUPS 20 works by reducing stomach acid. Cosec MUPS 20 blocks a specific enzyme (H+/K+ ATPase) in the stomach, which helps decrease acid production. After taking Cosec MUPS 20, the effect starts within one hour and reaches its peak in two hours, lasting up to 72 hours. Once stopped, acid production gradually returns to normal in about 3 to 5 days.
Dosage & Administration
Oral:
- Benign gastric and duodenal ulcers: 20 mg once daily for 4 weeks (duodenal) or 8 weeks (gastric); may increase to 40 mg daily if needed.
- NSAID-related ulcers: 20 mg once daily for 4 weeks, extend for another 4 weeks if not fully healed.
- GERD: 20 mg once daily for 4 weeks; if needed, continue for 4-8 weeks. 40 mg once daily for severe cases.
- Long-term acid reflux management: 10-20 mg daily.
- Indigestion: 10-20 mg once daily for 2-4 weeks.
- Prophylaxis for acid aspiration during surgery: 40 mg on the night before surgery, then 40 mg 2-6 hours before surgery.
- Zollinger-Ellison syndrome: Start with 60 mg once daily, up to 120 mg daily, divided into 2 doses if necessary.
- Helicobacter pylori eradication: 20 mg twice daily with antibiotics (Amoxicillin and Metronidazole or Clarithromycin) for one week.
- For children over 1 year (severe reflux esophagitis): 10-40 mg daily based on weight for 4-12 weeks.
IV Injection:
- Prophylaxis for acid aspiration: 40 mg IV 1 hour before surgery.
- Duodenal, gastric ulcers, or reflux esophagitis: 40 mg IV once daily if oral medication is not suitable.
- Zollinger-Ellison syndrome: Start with 60 mg IV daily. Adjust dose as necessary.
Interaction
Cosec MUPS 20 can reduce the absorption of certain medications like ketoconazole. Cosec MUPS 20 may also slow the breakdown of drugs like diazepam, phenytoin, and warfarin, so monitoring of these drugs is advised. Omeprazole and clarithromycin may increase each other’s levels when used together. There are no significant interactions with other drugs like caffeine, cyclosporin, or aspirin. Alcohol or food doesn’t affect its absorption.
Contraindications
Cosec MUPS 20 is not recommended for people who are allergic to Cosec MUPS 20. If there is a suspicion of a gastric ulcer being cancerous, Cosec MUPS 20 should be ruled out before starting treatment.
Side Effects
Cosec MUPS 20 is generally well-tolerated, with mild and reversible side effects. Some people may experience rash, itching, diarrhea, headache, or stomach discomfort. Rarely, more serious effects like dizziness, confusion, or liver problems can occur. Most side effects go away once treatment is stopped.
Pregnancy & Lactation
Studies show that Cosec MUPS 20 is safe during pregnancy, but consult your doctor before using Cosec MUPS 20. Cosec MUPS 20 is unknown if Cosec MUPS 20 passes into breast milk, so breastfeeding should be stopped if necessary to take the drug.
Precautions & Warnings
Cosec MUPS 20 should not be used with clopidogrel (a blood thinner). Long-term use may increase the risk of bone fractures or lead to stomach issues. People with liver disease or kidney problems should be cautious and consult their doctor.
