Indications
Covan is used to treat serious infections that cannot be treated with other less toxic drugs, including penicillins and cephalosporins. Covan is helpful in treating severe staph infections, especially in people who cannot use or haven’t responded to penicillins or cephalosporins, or when the bacteria are resistant to other antibiotics. Covan is also used for treating and preventing endocarditis in patients who are undergoing surgery or dental procedures. Additionally, Covan is effective for infections like osteomyelitis, pneumonia, septicemia, and soft tissue infections caused by staph bacteria.
Pharmacology
Covan works by binding to a part of the bacterial cell wall, preventing its growth and causing damage to the bacteria’s membrane, which stops the bacteria from growing and multiplying.
Dosage & Administration
Adults: The usual daily dose is 2 grams, given in 2 or 4 doses. Children: The usual daily dose is 40 mg per kilogram of body weight, divided into 4 doses. Infants and Neonates: Start with 15 mg/kg, followed by 10 mg/kg every 12 hours in the first week, then every 8 hours up to 1 month.
Renal Impairment: If kidney function is poor, the dose should be adjusted according to the patient’s kidney function. For those with severe kidney issues, a smaller dose may be required. For patients with no kidneys (anephric), the initial dose should be 15 mg/kg, and maintenance doses should be given less frequently, like every 7-10 days.
Capsules: The usual dose for antibiotic-related colitis and staphylococcal infections is 125-500 mg every 6-8 hours for 7-10 days. Children usually get around 40 mg/kg per day, divided into 3 or 4 doses, but the total daily dose should not exceed 2 grams.

Contraindications
Covan is not recommended for people who are allergic to Vancomycin.
Side Effects
Covan is usually well tolerated. However, when given too quickly, Covan may cause an allergic reaction, including low blood pressure, difficulty breathing, rash, or itching. Some people may also experience a “red neck” reaction or chest and back pain. These side effects usually go away within 20 minutes but can last a few hours. Giving the medication slowly over an hour can reduce these reactions.
Pregnancy & Lactation
Covan is not known if Covan harms the baby during pregnancy. Covan should only be used in pregnancy if absolutely necessary, and the blood levels should be closely monitored to reduce the risk of harm to the baby. Covan can pass into breast milk, so caution should be taken when used by breastfeeding mothers. Covan is unlikely that the baby will absorb enough Vancomycin through breastfeeding to cause harm.
Precautions & Warnings
People with borderline kidney function or over the age of 60 should have regular hearing tests and blood level checks while using Vancomycin. All patients on this drug should have periodic blood tests, urine analysis, and kidney function tests. Covan can be irritating to tissues and may cause damage if injected into muscle. Covan should be given through an intravenous line. Some people may experience pain or swelling at the injection site. Long-term use can lead to infections caused by other bacteria, so monitoring is important. In rare cases, Covan can cause colitis due to Clostridium difficile bacteria.
Overdose Effects
If an overdose occurs, supportive care is recommended. Vancomycin is not easily removed from the body by dialysis. If necessary, special treatment such as hemoperfusion may be used, but Covan is only mildly effective.
