Indications
Coxpar 40 is used for short-term relief of pain after surgery in adults. The decision to use this medication should be based on an assessment of the patient’s individual risks.
Pharmacology
Coxpar 40 is a prodrug that turns into valdecoxib in the body. Valdecoxib is a selective COX-2 inhibitor, which works by blocking the COX-2 enzyme responsible for producing prostaglandins, substances that cause pain, swelling, and fever. COX-2 also plays a role in other body functions like regulating kidney function and influencing pain perception. This medication might also help with ulcer healing, though its role in this area isn’t fully confirmed.
Dosage & Administration
Adult: The recommended dose is 40 mg given through an injection, either intravenously (IV) or intramuscularly (IM), followed by 20 to 40 mg every 6 to 12 hours as needed, but not exceeding 80 mg per day. Coxpar 40 is advised to use the lowest dose for the shortest duration possible due to cardiovascular risks associated with higher doses.
Elderly: No dose adjustment is needed for elderly patients, though those weighing less than 50 kg should start with half the normal dose and not exceed 40 mg per day.
Children: Coxpar 40 is not recommended for children under 18 years as its safety and effectiveness have not been established.

Contraindications
Coxpar 40 is contraindicated for people who are allergic to the active ingredient or any of the other components of the medication.
Side Effects
Common side effects include changes in blood pressure, back pain, swelling in the ankles or legs, numbness or loss of sensation, stomach issues like nausea and bloating, abnormal kidney function, dizziness, difficulty sleeping, and increased sweating. There may also be a risk of anemia, sore throat, difficulty breathing, skin itching, and reduced urine output. Rarely, dry socket can occur after tooth extractions.
Pregnancy & Lactation
If pregnant or breastfeeding, Coxpar 40 is important to consult a doctor before using this medication. Coxpar 40 is not recommended during the third trimester of pregnancy due to risks of birth defects and complications. Parecoxib can transfer into breast milk and should not be used by nursing mothers.
Precautions & Warnings
Coxpar 40 is important to use caution with parecoxib, especially in patients who may develop iron overload or have certain medical conditions. Patients should be monitored for side effects, and if there is no improvement, other treatment options should be considered. Overdose Effects
In case of overdose, patients should receive symptomatic and supportive care. Valdecoxib, the active metabolite, is not removed by hemodialysis. Urine alkalization may not be useful due to its high protein binding.
