Indications

Cytocarb is used for the initial treatment of advanced ovarian cancer, especially in combination with other chemotherapy drugs. Cytocarb is also used to relieve symptoms in patients with recurrent ovarian cancer after previous chemotherapy.

Pharmacology

Cytocarb is an alkylating agent that attaches to DNA. Cytocarb disrupts the structure and function of DNA, which affects the cell cycle.

Dosage & Administration

Do not use needles or intravenous sets that contain aluminum parts for preparing or giving the injection. Aluminum reacts with the drug and may affect its strength. After dilution, Cytocarb should be given only through an intravenous route. The recommended dosage for adults with normal kidney function is 400 mg/m² as a single dose, infused over 15 to 60 minutes. Therapy should be repeated only after four weeks and if blood counts are within safe levels. For older patients or those with health concerns, dosage adjustments may be needed. Cytocarb is important to monitor blood counts during treatment.

Interaction

Cytocarb should not be mixed with other drugs. Other neurotoxic drugs may increase the harmful effects of Cytocarb on the kidneys.

Contraindications

Cytocarb should not be used in patients with severe kidney problems, unless the doctor believes the benefits outweigh the risks. Cytocarb should not be used in patients with severe bone marrow suppression or severe allergic reactions to Cytocarb, other platinum compounds, or mannitol. Cytocarb is also contraindicated for patients with bleeding tumors.

Side Effects

The side effects reported are based on data from many patients with various health conditions.

Hematologic toxicity: Bone marrow suppression is the main side effect. Low platelet count occurs in 25% of patients. Low white blood cell count occurs in 18%, and low red blood cell count occurs in 71%. These effects are usually temporary, but may be more severe for patients with kidney issues or poor health.

Nephrotoxicity: Kidney issues occur in some patients, with mild symptoms like higher creatinine or blood urea levels. Kidney function tests should be closely monitored.

Gastrointestinal toxicity: About 15% of patients experience nausea without vomiting, and 65% have vomiting. Nausea and vomiting usually go away within 24 hours. Other gastrointestinal side effects include pain, diarrhea, constipation, and loss of appetite.

Allergic reactions: Less than 2% of patients report allergic reactions, including rash, fever, itching, and rarely, breathing problems or low blood pressure.

Liver toxicity: Mild liver test abnormalities are common but usually reversible.

Neurotoxicity: Peripheral neuropathy (numbness or tingling in the hands and feet) affects 4% of patients, especially those who have had previous treatment with cisplatin.

Pregnancy & Lactation

Cytocarb is categorized as Pregnancy Category D, meaning Cytocarb may harm an unborn baby. There are no well-controlled studies in pregnant women, and Cytocarb can be found in human milk.

Precautions & Warnings

Cytocarb should be given under the care of a doctor experienced with cancer treatments. Blood counts, kidney, and liver function should be closely monitored. The drug should be stopped if severe side effects are observed. Patients with kidney problems or other drugs that affect the kidneys may experience worse side effects. The drug should not be given more often than every four weeks.
