Indications
D-Dopamine is used to treat low blood pressure or shock caused by heart attacks, infections, injuries, and kidney failure. D-Dopamine can also help after heart surgery when low blood pressure continues after fluid treatment. D-Dopamine is also used in long-term heart problems like heart failure.
Pharmacology
D-Dopamine is a medicine made from Dopamine hydrochloride, which works by stimulating different receptors in the body. Dopamine is naturally made in the body and is important for nerve signaling, especially in the brain. D-Dopamine helps increase heart rate and heart strength, improving blood flow and kidney function. D-Dopamine doesn’t affect the brain directly since D-Dopamine can’t pass through the blood-brain barrier.
Dosage & Administration
Adults: First, fluids should be given to restore blood volume. Start with a dose of 2 to 5 micrograms per kg per minute for those who respond to small increases in heart strength and kidney flow.
If the patient is more seriously ill, the dose should be increased gradually to 20 to 50 micrograms per kg per minute. If higher doses are needed, urine output should be monitored closely.
Children under 12: The safety of this drug for children under 12 hasn’t been established.
Older patients: No special dose changes are needed, but careful monitoring is necessary for blood pressure and urine flow.
Interaction
D-Dopamine works better with monoamine oxidase inhibitors (MAOIs). Using D-Dopamine with certain anesthetics may cause irregular heartbeats. Beta blockers like Propranolol and Metoprolol may reduce its effects. Other medicines that affect blood pressure or heart function may interact with D-Dopamine, so D-Dopamine’s important to check with a doctor before using them together.
Contraindications
D-Dopamine is not safe for people who are allergic to D-Dopamine or have certain heart problems like uncorrected fast heart rhythms or severe arrhythmias. D-Dopamine should not be used with dextrose solutions and blood through the same infusion line due to potential red blood cell clumping.
Side Effects
Common side effects include abnormal heartbeats, nausea, vomiting, fast heart rate, chest pain, trouble breathing, headache, and changes in blood pressure. Other less common effects are low heart rate, skin reactions, and high blood pressure. Severe issues like poor circulation in patients with vascular disease can also occur.
Pregnancy & Lactation
Studies in animals show no harm to the fetus, but D-Dopamine should only be used during pregnancy if the doctor decides the benefits outweigh the risks. D-Dopamine is not known if D-Dopamine passes into breast milk, so D-Dopamine’s not recommended for breastfeeding unless absolutely necessary.
Precautions & Warnings
D-Dopamine should not be given to patients with certain heart problems. Patients on monoamine oxidase inhibitors (MAOIs) will need a much smaller dose. Care should be taken in patients with kidney or liver problems. Excessive doses can cause electrolyte imbalances, and the infusion should be stopped gradually to avoid a sudden drop in blood pressure.
Overdose Effects
Overdose symptoms may include very high blood pressure. The infusion rate should be reduced or stopped until the patient stabilizes. If this doesn’t work, other medications like Phentolamine may be used to help control the situation.
