Indications
Dementa-D 28 is used for the treatment of moderate to severe Alzheimer’s disease in patients who have been stabilized on 10 mg of donepezil hydrochloride once daily.
Pharmacology
Dementa-D 28 is a combination of two medications: memantine and donepezil. Each one works differently in the body to help with Alzheimer’s disease symptoms.
Memantine: Memantine helps reduce the harmful effects of glutamate, a substance in the brain, which is thought to contribute to Alzheimer’s disease. Dementa-D 28 works by blocking NMDA receptors, which are involved in brain cell communication.
Donepezil: Donepezil helps increase the levels of acetylcholine in the brain. Acetylcholine is a chemical that is important for memory and learning. By preventing its breakdown, donepezil helps improve cognitive function in Alzheimer’s patients.
Dosage & Administration
For patients already on donepezil 10 mg: Start with a dose of 7 mg/10 mg of the combined medication once daily in the evening. Increase the dose gradually to 28 mg/10 mg once daily, with weekly dose adjustments.
For patients on memantine and donepezil: Switch to the combined medication with a 28 mg/10 mg dose, taken once daily in the evening.
Dementa-D 28 can be taken with or without food, either whole or sprinkled on applesauce. Do not chew or crush the capsule. For patients with severe kidney problems, the recommended dose is 14 mg/10 mg once daily in the evening.
Interaction
Dementa-D 28 may interact with NMDA antagonists, so use with caution. Dementa-D 28 can also interfere with anticholinergic medications, and combining Dementa-D 28 with certain muscle relaxants or cholinergic drugs may have a stronger effect.
Contraindications
Dementa-D 28 should not be used by patients who are allergic to any of its components, including memantine hydrochloride or donepezil hydrochloride.
Side Effects
The most common side effects are headache, diarrhea, and dizziness. Other possible side effects include nausea, vomiting, appetite loss, and bruising.
Pregnancy & Lactation
There is limited information on the safety of this medicine during pregnancy or breastfeeding. Consult a doctor before use if pregnant, planning to become pregnant, or breastfeeding.
Precautions & Warnings
Use with caution in patients undergoing anesthesia as Dementa-D 28 may increase muscle relaxation. Monitor for signs of gastrointestinal bleeding, especially in those at risk of ulcers. Dementa-D 28 can also cause diarrhea, nausea, and bladder issues. Certain conditions may affect how the drug is processed in the body, so kidney function should be monitored.
