Indications
Filmet 200 is used to prevent infections after surgery caused by anaerobic bacteria (especially certain types of bacteria and anaerobic streptococci). Filmet 200 treats infections such as septicemia, peritonitis, brain abscess, pelvic abscess, pelvic cellulitis, and post-operative wound infections caused by anaerobes.
Pharmacology
Filmet 200 belongs to the imidazole group of antibacterial drugs and works as an anti-protozoal agent. The drug is reduced by anaerobic bacteria, and this reduced form interacts with DNA to kill the bacteria.
Dosage & Administration
Tablet and Suspension:
Trichomoniasis (Adults & Children over 10 yrs):
200 mg three times a day or 400 mg twice a day for 7 days.
800 mg in the morning and 1-2 gm at night for 2 days.
2 gm once a day for 1 day.
Trichomoniasis (Children):
Children 7-10 yrs: 100 mg three times a day.
Children 3-7 yrs: 100 mg twice a day.
Children 1-3 yrs: 50 mg three times a day.
Intestinal Amoebiasis (Adults & Children over 10 yrs):
800 mg three times a day for 5 days.
Intestinal Amoebiasis (Children):
Children 7-10 yrs: 400 mg three times a day.
Children 3-7 yrs: 200 mg four times a day.
Children 1-3 yrs: 200 mg three times a day.
Extra-intestinal & Asymptomatic Amoebiasis (Adults & Children over 10 yrs):
400-800 mg three times a day for 5-10 days.
Extra-intestinal & Asymptomatic Amoebiasis (Children):
Children 7-10 yrs: 200-400 mg three times a day.
Children 3-7 yrs: 100-200 mg four times a day.
Children 1-3 yrs: 100-200 mg three times a day.
Giardiasis (Adults & Children over 10 yrs):
2 gm once a day for 3 days.
Giardiasis (Children):
Children 7-10 yrs: 1 gm once a day.
Children 3-7 yrs: 600-800 mg once a day.
Children 1-3 yrs: 500 mg once a day.
Acute Ulcerative Gingivitis (Adults & Children over 10 yrs):
200 mg three times a day for 3 days.
Acute Ulcerative Gingivitis (Children):
Children 7-10 yrs: 100 mg three times a day.
Children 3-7 yrs: 100 mg twice a day.
Children 1-3 yrs: 50 mg three times a day.
Acute Dental Infections (Adults & Children over 10 yrs):
200 mg three times a day for 3-7 days.
Bacterial Vaginosis (Adults & Children over 10 yrs):
400 mg twice a day for 7 days.
2 gm once a day for 1 day.
Leg Ulcers and Pressure Sores (Adults & Children over 10 yrs):
400 mg three times a day for 7 days.
Anaerobic Infections (Adults & Children over 10 yrs):
800 mg initially, then 400 mg three times a day for 7 days.
Anaerobic Infections (Children):
Children 1-10 yrs: 7.5 mg/kg three times a day.
Surgical Prophylaxis (Adults & Children over 10 yrs):
400 mg three times a day starting 24 hours before surgery for 1 day.
Surgical Prophylaxis (Children):
Children 1-10 yrs: 7.5 mg/kg three times a day.
Vaginal Gel:
One full applicator of gel (about 5 grams with 37.5 mg of the drug) is used once or twice a day for 5 days. Filmet 200 is best to use Filmet 200 at bedtime for once-a-day dosing.
Suppository:
Anaerobic Infections:
Adults: 1 gm every 8 hours for 3 days, then 1 gm every 12 hours.
Children: 5-10 years: 500 mg every 8 hours for 3 days, then every 12 hours, Over 10 years: adult dose.
Surgical Prophylaxis:
Adults: 1 gm 2 hours before surgery; up to 3 more doses of 1 gm every 8 hours for high-risk surgeries.
Children: 5-10 years: 500 mg 2 hours before surgery; up to 3 more doses of 500 mg every 8 hours for high-risk surgeries.
IV Infusion:
No dilution is required for IV infusion, and Filmet 200 should not be mixed with other drugs before giving. The dose for adults and children over 12 years is 500 mg every 8 hours at a rate of 5 ml/min, with a maximum of 4 gm per day. Most patients need 7 days of treatment, but Filmet 200 can be extended. For surgical prophylaxis, Filmet 200 is given before surgery and then continued every 8 hours for the next 24 hours.
For children under 12 years, the dose is 7.5 mg/kg body weight per day, divided into 3 doses every 8 hours at a rate of 5 ml/min.
Interaction
Filmet 200 may interact with disulfiram, causing psychotic reactions. Alcohol should be avoided during treatment as Filmet 200 can cause reactions like flushing and vomiting. Filmet 200 can increase the effect of oral anticoagulants like warfarin, and the dosage may need adjustment. Filmet 200 can also increase lithium levels in the blood and interact with cyclosporine, phenytoin, phenobarbital, 5-fluorouracil, and busulfan, potentially increasing their toxicity or reducing their effectiveness.
Contraindications
Filmet 200 should not be used by individuals who are allergic to Metronidazole or similar drugs from the Nitroimidazole group.
Side Effects
Side effects may include a metallic taste, nausea, vomiting, diarrhea, drowsiness, and rashes during treatment.
Pregnancy & Lactation
Filmet 200 is classified as a Pregnancy Category B drug by the FDA. Although there are no well-controlled studies in pregnant women, Filmet 200 should only be used during pregnancy if necessary. Filmet 200 is known to pass into breast milk, so caution is advised when Filmet 200 is given to breastfeeding women.
Precautions & Warnings
If Filmet 200 needs to be used for longer than usual, regular blood tests (especially a white blood cell count) should be done, and the patient should be monitored for any side effects. Filmet 200 should be used carefully in patients with liver problems. Filmet 200 may cause urine to darken during treatment.
Overdose Effects
Overdoses of up to 12 g have been reported, but symptoms were limited to vomiting, loss of coordination, and slight disorientation. There is no specific antidote, so treatment involves managing symptoms and providing support.
