Indications

Milran 50 is used to treat fibromyalgia. Milran 50 is not approved for use in children.

Pharmacology

Milran 50 is a strong medicine that blocks the reuptake of norepinephrine and serotonin in the brain. Milran 50 works more strongly on norepinephrine than serotonin and doesn’t directly affect dopamine or other brain chemicals.

Dosage & Administration

Adult: The usual dose is 100 mg per day (50 mg twice a day). The dose can be adjusted based on how well the medicine works and how well the patient tolerates Milran 50. The dose schedule is as follows:

Day 1: 12.5 mg once

Days 2-3: 25 mg/day (12.5 mg twice daily)

Days 4-7: 50 mg/day (25 mg twice daily)

After Day 7: 100 mg/day (50 mg twice daily)

The dose can be increased to 200 mg/day (100 mg twice daily) depending on how the patient responds. Doses higher than 200 mg/day have not been tested.

Interaction

Taking Milran 50 with aspirin, NSAIDs, warfarin, or other drugs that affect blood clotting can increase the risk of bleeding. Milran 50 can also cause stronger effects when taken with certain medications like clomipramine, other serotonin-related drugs, and antipsychotics. Milran 50 may reduce the blood pressure-lowering effect of clonidine and can cause problems if taken with digoxin. Taking Milran 50 with epinephrine or norepinephrine can lead to high blood pressure and heart rhythm problems.

Contraindications

Milran 50 should not be used by people with uncontrolled narrow-angle glaucoma or anyone taking MAO inhibitors, or who have recently stopped using them (within 2 weeks).

Side Effects

Common side effects include increased heart rate, high blood pressure, liver problems, low sodium, unusual bleeding, difficulty urinating, enlarged pupils, nausea, vomiting, constipation, headaches, trouble sleeping, dizziness, hot flashes, excessive sweating, palpitations, dry mouth, and migraines.

Pregnancy & Lactation

Pregnancy: If you are pregnant or plan to become pregnant, let your doctor know before taking Milran 50. If you become pregnant while taking Milran 50, you should join the Milnacipran Pregnancy Registry to help gather information about its safety during pregnancy.

Lactation: If you are breastfeeding, inform your doctor before continuing treatment.

Precautions & Warnings

Patients with major depression, other psychiatric conditions, or a history of certain conditions like bladder issues, glaucoma, high blood pressure, heart problems, or seizures should be cautious. Do not stop taking Milran 50 suddenly, especially if you have liver or kidney problems. Milran 50 should be avoided during pregnancy and breastfeeding unless advised by a doctor.

Overdose Effects

Signs of overdose include high blood pressure, heart and breathing problems, confusion, dizziness, and liver issues. Immediate treatment with gastric lavage and activated charcoal is needed. Supportive care such as oxygen, monitoring heart rhythm, and temperature control may be required.

Therapeutic Class

Milran 50 is classified as a serotonin-norepinephrine reuptake inhibitor (SNRI).

Storage Conditions

Store at 25°C, away from light and moisture, and keep Milran 50 out of the reach of children.
