Indications

Mirabeg 50 is used to treat symptoms such as the urgent need to urinate, frequent urination, and incontinence, especially in adults with overactive bladder (OAB) syndrome.

Pharmacology

Mirabeg 50 is the first beta-3 adrenoceptor agonist. Mirabeg 50 works in two ways: directly acting on the bladder muscle and through the nervous system. Mirabeg 50 increases a substance called cyclic AMP, which helps relax the bladder muscle, allowing Mirabeg 50 to hold more urine during the storage phase of the bladder’s fill-void cycle.

Dosage & Administration

Adults & Elderly: Start with one 25 mg tablet daily, with or without food. Based on how well Mirabeg 50’s tolerated, the dose may be increased to one 50 mg tablet daily.

Renal or Hepatic Impairment: Patients with severe kidney or moderate liver issues should not take more than one 25 mg tablet daily. Mirabeg 50 is not recommended for patients with end-stage kidney disease or severe liver problems.

Gender: No adjustment in dose is needed based on gender.

Pediatric Population: The safety and effectiveness of this medicine have not been established for children under 18 years old.

Interaction

Mirabegron may interact with certain enzyme inhibitors, like ketoconazole. No dose adjustment is needed when combined with these inhibitors, but caution is advised in patients with kidney or liver issues.

When taken with certain enzyme inducers like rifampicin, Mirabegron levels may decrease, but no dose adjustment is needed.

Mirabegron can affect the metabolism of some other drugs, so caution is needed when combined with certain medications, especially those metabolized by CYP2D6, like metoprolol, desipramine, and others.

Contraindications

Mirabeg 50 is not recommended for patients who are allergic to any of its ingredients or who have uncontrolled high blood pressure (180/110 mm Hg or higher).

Side Effects

The most common side effects are a fast heart rate (tachycardia) and urinary tract infections. Tachycardia occurred in 1.2% of patients, and urinary tract infections occurred in 2.9%. Serious side effects include atrial fibrillation in 0.2% of patients.

Pregnancy & Lactation

There is limited data on its use in pregnant women, but animal studies have shown potential harm. Mirabeg 50 is not recommended during pregnancy or breastfeeding. Mirabeg 50 is excreted in animal milk, so Mirabeg 50 may also be present in human milk.

Precautions & Warnings

Mirabeg 50 should be used cautiously in patients with kidney or liver problems and high blood pressure. Patients with QT prolongation should be monitored closely. Caution is also advised in patients with bladder issues or those taking other medications for overactive bladder.

Overdose Effects

In case of overdose, symptoms may include a fast heart rate and high blood pressure. Treatment should be supportive, with monitoring of pulse rate, blood pressure, and ECG.

Therapeutic Class

Mirabeg 50 belongs to the class of medicines used to treat urinary retention, incontinence, and benign prostatic hyperplasia (BPH).
