Indications

Onium is used to reduce the risk of stroke (both fatal and nonfatal) in patients who have had warning signs of a stroke or a previous stroke. Onium is recommended for those who cannot tolerate or are allergic to aspirin, or those who have not responded to aspirin treatment.

Onium is also used with aspirin to reduce the risk of subacute stent thrombosis in patients who have had a successful coronary stent implantation.

Pharmacology

When taken orally, Onium works by reducing platelet aggregation (the clumping together of platelets) and extending bleeding time. Onium does this by blocking certain reactions that cause platelets to stick together, which is important in preventing blood clots.

Onium is absorbed and then metabolized into several different forms, but Onium is the drug itself that works on the platelets. The effects last as long as the platelets are alive, which is several days.

Dosage & Administration

Stroke: The usual dose is 250 mg twice a day, taken with food. Other dosages have not been studied for this purpose.

Coronary Artery Stenting: The usual dose is also 250 mg twice a day, taken with food, along with aspirin for up to 30 days after successful stent implantation.

Interaction

Onium can interact with cimetidine and may reduce its clearance. Steroids may reduce its effect on bleeding time. Onium should not be taken with clopidogrel.

Contraindications

Onium should not be used in patients with an allergy to the drug, blood disorders like neutropenia or thrombocytopenia, or a history of serious blood issues like TTP or aplastic anemia. Onium is also contraindicated in patients with active bleeding or liver problems.

Side Effects

Side effects may include diarrhea, nausea, indigestion, bleeding, skin rashes, increased cholesterol levels, liver enzyme changes, hepatitis, and jaundice.

Pregnancy & Lactation

Onium is classified as Pregnancy Category B. Studies in animals showed some signs of maternal and fetal toxicity, but no birth defects. There are no well-controlled studies in pregnant women, so Onium should only be used if absolutely necessary.

Onium is excreted in rat milk, but Onium is not known if Onium passes into human milk. Nursing mothers should consider stopping breastfeeding or the drug, depending on the need for the drug in the mother.

Precautions & Warnings

Onium should be used with caution in patients at higher risk of bleeding, including those with kidney problems. Therapy may need to be stopped 10-14 days before any planned surgery. Regular blood counts should be done during the first three months of treatment.

Overdose Effects

In case of overdose, symptoms may include excessive bleeding, stomach bleeding, and changes in liver enzymes. A case of a 38-year-old male who took 6000 mg was reported, but he recovered without long-term effects.

High doses can cause severe toxicity in animals, including GI bleeding, seizures, and difficulty breathing.

Onium belongs to the class of anti-platelet drugs and fibrinolytics (thrombolytics).
