Indications

It is used for the long-term maintenance treatment of airflow blockage in patients with chronic obstructive pulmonary disease (COPD), including chronic bronchitis and emphysema. It should not be used in sudden breathing issues or for asthma.

Pharmacology

It is an inhalation powder that combines Glycopyrronium (an anticholinergic) and Indacaterol (a LABA) for oral inhalation. Indacaterol works by increasing cyclic AMP in the body, which helps relax the bronchial muscles and reduces allergic reactions. Glycopyrronium blocks certain receptors in the airways, leading to easier breathing by relaxing the muscles around the airways.

Dosage & Administration

Adults (18 years or older): Use it for inhalation once a day at the same time each day. Do not exceed one dose every 24 hours.

Maintenance treatment for COPD: Inhale one dose daily.

Not for: Treating sudden breathing problems or asthma.

Geriatric: It can be used in elderly patients (65 years and older) at the recommended dose.

Use in children & adolescents: It is not for use in children or teenagers as its safety and effectiveness have not been proven.

Renal impairment: Can be used in patients with mild to moderate kidney problems. For severe kidney issues or dialysis patients, only use it if the benefits outweigh the risks.

Liver impairment: Can be used in patients with mild to moderate liver issues. Caution should be taken with severe liver impairment.

Interaction

No specific interaction studies were conducted for this combination. However, combining it with other beta-blockers, anticholinergics, or sympathomimetic drugs is not recommended. These drugs may worsen side effects. Caution should be taken if using it with treatments that lower potassium levels.

Contraindications

It should not be used in patients who are allergic to Glycopyrronium, Indacaterol, or any of its ingredients. It is also not recommended for people with severe milk protein allergies or asthma without a long-term asthma medication.

Side Effects

Possible side effects include heart issues (like rapid heartbeat), eye problems (like blurred vision), urinary retention, dry mouth, cough, and digestive issues. Some people may also experience allergic reactions like skin rash, swelling, or difficulty breathing. The most common side effects include cough and throat pain.

Pregnancy & Lactation

It is in pregnancy category C, which means there is no data on its safety in pregnant women. It should only be used if the benefits to the mother outweigh any potential risks to the baby. It is not known if this medication passes into breast milk, so it should only be used during breastfeeding if the benefits outweigh the risks to the infant.

Precautions & Warnings

It should not be used for sudden worsening of COPD or asthma. It should not be combined with other LABAs due to the risk of overdose. If breathing problems worsen, it should be stopped immediately. It should be used with caution in patients with heart disease, seizures, thyroid problems, diabetes, or high blood sugar. It can worsen conditions like glaucoma or urinary retention, so those with these issues should seek immediate medical advice if symptoms occur.

Overdose Effects

Overdosing could lead to severe symptoms such as rapid heartbeat, shaking, palpitations, headaches, nausea, or dizziness. Anticholinergic effects like increased eye pressure or difficulty urinating could also occur. In case of an overdose, supportive care should be given, and hospitalization may be required for severe cases.

It is a combined bronchodilator medication.
