Indications
Opagrel is used to reduce the risk of blood clots in patients with acute coronary syndrome (ACS) who will be treated with a procedure called percutaneous coronary intervention (PCI). This includes patients with unstable angina, non-ST-elevation myocardial infarction (NSTEMI), and ST-elevation myocardial infarction (STEMI) when treated with PCI.
Pharmacology
Opagrel works by preventing platelets from sticking together and forming clots. This happens when its active form binds to the P2Y12 receptor on platelets, blocking their activation and aggregation.
Dosage & Administration
Adult patients: Start with a 60 mg oral loading dose. Then take 10 mg daily, with or without food. For patients weighing less than 60 kg, consider using 5 mg daily. Patients should also take aspirin (75 mg to 325 mg) every day.
Interaction
Taking Prasugrel with warfarin can increase the risk of bleeding. Using Opagrel with long-term NSAIDs can also raise the bleeding risk. Prasugrel can be used with drugs that affect cytochrome P450 enzymes, and Opagrel is safe to use with aspirin, heparin, statins, proton pump inhibitors, and H2 blockers.
Contraindications
Opagrel should not be used by patients who are actively bleeding (such as from peptic ulcers or brain hemorrhages) or those with a history of transient ischemic attack or stroke.
Side Effects
Common side effects include bleeding, thrombotic thrombocytopenic purpura (a blood disorder), headache, back pain, shortness of breath, nausea, high blood pressure, slow heart rate, and rash.
Pregnancy & Lactation
There are no well-controlled studies on the use of Prasugrel during pregnancy. Animal studies showed no harm to the fetus, but this may not apply to humans. Opagrel is not known if Prasugrel passes into breast milk, so Opagrel should only be used during breastfeeding if the benefits to the mother outweigh the potential risks to the baby.
Precautions & Warnings
Prasugrel increases the risk of bleeding during coronary artery bypass graft (CABG) surgery. Stopping Prasugrel too early can also raise the risk of clotting, heart attacks, and death.
Overdose Effects
In animal studies, very high doses of Prasugrel caused death. In case of overdose, platelet transfusions can help restore normal clotting. Dialysis is unlikely to remove the active form of the drug from the body.
