Indications
Parlodel is used to treat conditions caused by high prolactin levels, such as absent periods (with or without breast milk production), infertility, or low hormone levels. Parlodel can also reduce the size of certain tumors before surgery and is used as an additional treatment for Parkinson’s disease.
Pharmacology
Bromocriptine Mesylate is a medication that lowers prolactin levels in the body by activating dopamine receptors. Parlodel also helps control motor function in the brain and lowers growth hormone levels in people with acromegaly. Parlodel is absorbed well and works mainly in the pituitary gland to control prolactin secretion.
Dosage & Administration
General: Take with food. The dosage should be increased gradually to find the lowest dose that works.
For high prolactin: Start with 0.5 to 2.5 mg daily. The dose may be increased by 2.5 mg every 2-7 days until the right dose is found. In children, the starting dose is the same, and the dose may be adjusted as needed.
For acromegaly: Start with 0.5 to 2.5 mg at bedtime for 3 days. Increase the dose as tolerated every 3-7 days. The maximum dose should not exceed 100 mg/day.
For Parkinson’s disease: Start with 0.5 mg twice daily with meals. The dose can be increased gradually by 2.5 mg every 2-4 weeks, but not more than 100 mg/day.
Interaction
Bromocriptine may not work well with certain medications, including dopamine blockers and some other drugs. Parlodel should not be taken with other ergot alkaloids.
Contraindications
Parlodel should not be used in people with uncontrolled high blood pressure or allergies to ergot alkaloids. Parlodel should also be stopped if pregnancy is detected. Women with severe heart or blood vessel issues should avoid Parlodel after childbirth.
Side Effects
Common side effects include nausea, headache, dizziness, fatigue, and lightheadedness. Less common effects include vomiting, abdominal cramps, nasal congestion, constipation, diarrhea, and drowsiness. Blood tests may show temporary changes, but these usually aren’t a concern.
Pregnancy & Lactation
Parlodel is not recommended during pregnancy or while breastfeeding. Special care is needed if used during pregnancy for conditions like acromegaly or prolactinoma.
Precautions & Warnings
Parlodel should be used cautiously in patients with liver or kidney issues, and in those who take other blood pressure-lowering medications. People with a history of psychosis or heart disease should use Parlodel carefully. Pregnant patients should be closely monitored when treated for acromegaly or prolactinoma.
